About this file

This document is a Vendor Disclosure Statement from the Colorado Department of Corrections requiring prospective vendors to disclose information about contract performance locations. The form is designed to capture details about where services will be performed, specifically if any work will occur outside of the United States or the State of Colorado. The disclosure is mandatory for contract solicitations, with specific exemptions for certain types of contracts including those under Medicare, Colorado Medical Assistance Act, Children Basic Health Plan Act, Colorado Indigent Care Program, and construction contracts.

The form requires vendors to provide the solicitation number, vendor name, and explicitly detail the counties and states where contract services will be performed. If services are anticipated to be performed outside of the United States or Colorado, vendors must explain why such external performance is necessary or advantageous. The form must be signed and dated by an authorized representative of the vendor, who will provide their printed name and title. This disclosure appears to be part of the Colorado state government's effort to ensure transparency and local economic consideration in government contracting processes.
