About this file
This document is an Offeror Disclosure Statement from the Colorado Department of Corrections for Solicitation Number IFB 2026000051. The form requires prospective vendors to disclose information about contract performance locations, specifically whether services will be performed outside of the United States or the State of Colorado. The disclosure is mandatory prior to contracting and applies to service contracts, with explicit exceptions for certain healthcare-related contracts and construction contracts.

The document provides space for the vendor's name, signature, printed name, and title, indicating it is a self-reported disclosure meant to provide transparency about the geographic scope of contract work.
