About this file

This is an Offeror Disclosure Statement form issued by the Colorado Department of Corrections for prospective vendors bidding on state contracts. The document requires vendors to disclose information regarding contract performance locations and any work to be performed outside the United States or the State of Colorado. Vendors must complete and return the disclosure statement to the contracting state agency prior to contract award, unless the contract falls under specific exemptions including Medicare, the Colorado Medical Assistance Act, the Children Basic Health Plan Act, the Colorado Indigent Care Program, or construction contracts. The form requires vendors to identify the solicitation number, vendor name, all counties and states where services will be performed under the contract and any subcontracts, and to provide detailed explanations if any work is anticipated outside the United States or Colorado.

The disclosure statement serves as a transparency and accountability mechanism for the state to track contract performance locations and ensure services are performed in alignment with state preferences and policy objectives. The form does not address pricing, set-asides, or funding sources, as it functions solely as a disclosure and compliance document. Vendors are responsible for completing all required fields with accurate information and providing signatures with dates and printed names and titles.
