About this file
This is a Vendor Disclosure and Notification form (Exhibit E) required by the Colorado Department of Corrections (CDOC) for the State of Colorado in connection with solicitation number DQ 2027000048. The form serves as a compliance document under Colorado Revised Statutes 24-102-206, requiring vendors to disclose whether any services under a contract or subcontracts will be performed outside the United States or the State of Colorado. Vendors must complete the form by answering whether services will be performed outside these jurisdictions and, if applicable, identifying the types of services, locations where services will be performed, and the justification for performing work outside the United States or Colorado. The form must be signed by the business President, Board Chairperson, or Contract's Authorized Signatory and returned with the bid submission to the Sole Contact listed in the solicitation.
The form includes procurement sections for internal use only, where the contracting agency documents the type of contract, the nature of services performed outside the U.S. or Colorado, and whether the vendor provided notice within the required 20-day timeframe of contracting or deciding to contract outside the specified jurisdictions. This disclosure requirement ensures transparency regarding offshore or out-of-state service delivery and allows the state to monitor compliance with Colorado procurement regulations regarding geographically dispersed contract performance.
