About this file

This is a Vendor Disclosure and Notification form issued by the Colorado Department of Corrections (CDOC) that addresses contract performance outside the United States or the State of Colorado, in accordance with Colorado Revised Statutes 24-102-206. The form serves as a mandatory disclosure document that vendors must complete and return with their bid submissions. The form requires vendors to disclose whether any services under the contract or subcontracts will be performed outside the United States or Colorado. If services will be performed outside these jurisdictions, vendors must specify the type of services, identify the country or state where services will be performed, and explain the necessity or business advantage of performing work outside the United States or Colorado. The form must be completed and signed by the business President, Board Chairperson, or the Contract's Authorized Signatory and returned to the sole contact listed in the solicitation.

The form includes a procurement-use-only section where the Colorado Department of Corrections will document the solicitation number, type of contract, type of services performed outside the U.S. or Colorado, and whether the vendor provided proper notice within 20 days of contracting or deciding to contract outside the United States or Colorado. This disclosure requirement ensures compliance with Colorado state law regarding vendor transparency and notification protocols for out-of-state or international service performance on government contracts.
