The form must be completed and returned with the bid by vendors bidding on the Boiler Chemicals & Treatment Services contract for Colorado Correctional Facilities (CCF). The document requires vendors to disclose whether any services, including subcontracted services, will be performed outside the United States or Colorado, and if so, to specify the type of services, geographic locations, and justification for performing work outside these jurisdictions. The form must be signed by an authorized representative such as the business president, board chairperson, or contract authorized signatory.

The form serves as official notice to the Colorado Department of Personnel in accordance with Colorado Revised Statutes 24-102-206. Vendors answering "No" to the question of whether services will be performed outside the U.S. or Colorado need only sign and return the form. Those answering "Yes" must provide detailed information regarding service types, specific countries or states where services will be performed, and a detailed explanation of the necessity or advantages of performing work outside the specified jurisdictions. The procurement office will document the vendor's responses, contract type, service types, and whether the vendor provided notice within 20 days of deciding to contract outside the U.S. or Colorado.
