About this file

This is a Vendor Disclosure and Notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, required to be completed and submitted with bids for non-construction contracts. The form serves as notice to the Department of Personnel when a vendor or vendor's subcontractor anticipates performing services outside the United States or the State of Colorado in accordance with Colorado Revised Statutes 24-102-205 and 24-102-206. Bidders must answer whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if "Yes," must provide the form completed with details regarding the nature of services, locations, and business justification for the external performance.

The form requires completion by the contractor's President, Board Chairperson, or Contract's Authorized Signatory and must be signed and returned to the Sole Contact listed in the solicitation. Required disclosures include the type of services to be performed outside the U.S. or Colorado (such as software, hosting, technical support, server monitoring, or design and development), the specific countries and states where services will be performed, and a written explanation of why performing services outside the jurisdiction is necessary or advantageous. The procurement office uses this form to verify whether the vendor provided timely notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado.
