About this file

This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, designed to ensure compliance with Colorado Revised Statutes regarding contract performance outside the United States or Colorado. The form must be completed and returned with bids for solicitation number DQ 2027000124, which is categorized as a Purchase Order. Bidders are required to disclose whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, must identify the specific types of services, geographic locations where performance will occur, and justification for performing work outside these jurisdictions. The form must be signed by an authorized representative such as the business president, board chairperson, or contract authorized signatory.

The form includes a procurement section requiring documentation that vendors provide written notice within 20 days of contracting or deciding to perform services outside the US or Colorado. Vendors must fully describe all services to be performed externally, such as software development, hosting, technical support, server monitoring systems, or design services. The disclosure requirements are mandatory for all non-construction contracts, and failure to properly complete and return this form with the bid submission may result in bid rejection or contract complications. This exhibit serves as official notice to the Department of Personnel of any offshore or out-of-state performance arrangements.
