About this file

This is a vendor disclosure and notification form required by the Colorado Department of Corrections (CDOC) for the State of Colorado in connection with solicitation number DQ 2027000108, a purchase order for fresh eggs. The form must be completed and returned by vendors as part of their bid submission. If services will be performed outside Colorado or the U.S., vendors must describe the service type (such as software, hosting, technical support, or design and development), identify all countries and states where performance will occur, and explain the necessity or advantage of conducting work outside the United States or Colorado. The procurement office must verify that vendors provided the required notice within 20 days of deciding to contract outside the U.S. or Colorado, ensuring compliance with Colorado Revised Statutes 24-102-205 and 24-102-206.
