About this file

This is a Bid/Proposal Affidavit form issued by the Maryland Judiciary's Procurement Office for use in connection with the Administrative Office of the Courts' Request for Proposals to provide consulting services for mapping data-related business processes across all circuit courts in Maryland. The affidavit serves as a certification document that bidders must complete and submit with their proposals, confirming the authorized representative's legal authority to act on behalf of the bidding business entity.

The affidavit requires bidders to affirm compliance with multiple Maryland statutory requirements and federal regulations, including certifications that neither the bidder nor its officers, directors, members, partners, controlling stockholders, or employees involved in contracting activities have been convicted of bribery, fraud, embezzlement, antitrust violations, RICO violations, mail fraud, minority business enterprise law violations, commercial nondiscrimination violations, or any related conspiracy charges. Bidders must also affirm that they have not been suspended or debarred by any public entity, that the business was not established to evade debarment provisions, that no debarred or suspended persons will provide services under the contract, that no collusion or bid-fixing has occurred, and that all contingent fees have been disclosed. Additionally, bidders must certify that the business is registered in good standing with the Maryland State Department of Assessments and Taxation, has paid all state taxes, and will comply with Maryland's election law regarding political contributions for contracts exceeding $200,000 in cumulative consideration.
