This is Addendum No. 1 to a Request for Proposals issued by the City of Chandler, Arizona for an Employee Assistance Program (EAP) solicitation number HR5-918-4886, with a proposal due date of March 26, 2025. The City seeks to contract with a qualified EAP provider to deliver comprehensive support services to municipal employees. The contract is structured as a one-year agreement with four optional one-year renewals. The current workforce comprises 1,770 eligible employees working in a hybrid environment, with 85 employees subject to Department of Transportation regulations. The addendum clarifies that the current EAP contract is not hours-based but includes onsite training, orientation, educational seminars, health fair participation (with the City considering hosting two health fairs annually at four hours each), onsite critical incident support, and webinar training. Electronic signatures are acceptable on all proposal forms. The addendum also specifies that offerors are not required to name specific counselors but must provide counts as requested in the RFP.
The current EAP provider is United Behavioral Health, with the existing agreement effective January 1, 2021, at a rate of $2.29 Per Employee Per Month (PEPM) for the first three years (2021-2023), with 3 percent escalation applied in 2024 and 2025. Vendors must guarantee pricing for the first three years of any new agreement. The addendum confirms that Department of Transportation Substance Abuse Professional (SAP) evaluations are not included in the EAP contract and are provided on a fee-for-service basis. Insurance requirements have been substantially modified, including Commercial General Liability of $2,000,000 per occurrence and $4,000,000 aggregate (with $1,000,000 per occurrence, $2,000,000 aggregate, plus $5,000,000 umbrella coverage acceptable), Automobile Liability of $1,000,000 each accident, Workers Compensation and Employers Liability of $1,000,000 per accident and per disease, Professional Liability of $1,000,000 each claim and $2,000,000 all claims, and Technology Errors and Omissions Liability with minimum limits of $5,000,000 per loss and $5,000,000 aggregate.
