SOW Paving at Back Gate

NATIONAL MINE HEALTH AND SAFETY ACADEMY

STATEMENT OF WORK

The National Mine Health and Safety Academy (Academy), located at 1301 Airport Road, Beaver, WV, is seeking a vendor to provide labor and materials for preparation and installation of asphalt pull-off lanes (450 square yards) along the Mine Academy Drive access road at the National Mine Health and Safety Academy. (See specific tasks in Part 5) 1.1

OBJECTIVES

The objective of this procurement is to develop upgrades necessary to improve traffic flow and safety at Mine Academy Drive access point to the facility.

1.2

SCOPE

The Contractor shall provide labor and materials to construct full-depth pavement to widen 230 linear feet of an existing asphalt access road. The widened access road is to provide additional storage lanes for vehicle processing during high-traffic events at the Academy. The paved area shall be approximately 20 feet wide. The total surface area of the additional storage lanes will be 450 square yards.

1.2.1

SPECIAL QUALIFICATIONS

The contractor warrants that he will have obtained all licenses and permits required by law to engage in the activities that are required in connection with the statement of work. The contractor shall conform to all required regulations for the Academy, County, State, and the Federal Government including, but not limited to, safety and environmental.

1.5.1 CONTRACTING OFFICER REPRESENTATIVE: The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communications with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, and specifications; monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting contract.

1.9

PLACE OF PERFORMANCE:

The work to be performed under this contract will be at 1301 Airport Road, Beaver, WV.

1.10

PERIOD OF PERFORMANCE

Work shall begin upon contract award and shall be completed no later than

September 15, 2013.

1.11

SECURITY

It is the policy of the United States to enhance security, increase Government efficiency, reduce identity fraud, and protect personal privacy by a mandatory, Government-wide standard for secure and reliable forms of identification issued by the Federal Government to its employees and contractors (including contractor employees).

In conjunction with this, the following forms must be submitted to the Administrative Officer at the National Mine Health and Safety Academy within three days after award of the contract:

DEFINITIONS & ACRONYMS

CO – Contracting Officer

COR - Contracting Officer’s Representative: A representative from the requiring activity assigned by the Contracting Officer to perform surveillance and to act as liaison to the contractor.

DEFECTIVE SERVICE - A service output that does not meet the standard of performance associated with it in the Statement of Work.

PART 3

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

GOVERNMENT FURNISHED ITEMS AND SERVICE

Knowledgeable MSHA personnel will be made available to direct the contractor to the area to be prepped and paved. Reasonable amounts of electricity and potable water will be made available to the contractor without charge. Toilet facilities for contractor employees located adjacent to the garage bays in F building will be made available.

3.1

CONTRACTOR USE OF PREMISES

The contractor will be performing this work on the grounds of the National Mine Health and Safety Academy. The grounds have controlled access and as such the contractor shall comply with Academy security procedures.

The contractor will be provided a storage / staging area for equipments and materials in an area determined by the COR. The contractor is responsible for restoring the area to its original condition at the completion of the work and repairing any damages incurred to the area as a result of contractor activities.

The contractor is responsible for the removal and disposal of waste that is a result of this paving project and must comply with all federal, state and local regulations pertaining to the transport and disposal of waste.

3.2

WORK HOURS

Work hours for the contractor shall be between the hours of 7:00 AM and 5:00 PM, Monday - Friday

PART 4

CONTRACTOR FURNISHED ITEMS AND SERVICES

4.0 Except for those items, services, or commodities specifically stated in Part 3 as government furnished, the contractor must furnish everything needed to perform this contract according to all its terms.

PART 5

SPECIFIC TASKS

The following is the set of tasks required under this Contract:

5.1

WORK REQUIREMENTS

The intent of this project is to widen approximately 230 linear feet of an existing access road to the Academy by 20 feet.

5.2

QUALITY ASSURANCE

The contractor shall submit past performance references for prime and all sub-contractors. Past performance shall be related to this scope.

The contractor shall make a physical inspection of the project site prior to preparing an offer in order to accurately assess the level of work required.

5.3

SUBMITTALS

Contractor shall furnish submittals to COR for approval before purchasing material. Seek guidance from COR for details and specifics of below plans.

5.4

WORK ITEMS

1. The contractor shall construct approximately 450 square yards of full-depth pavement for additional vehicle storage. The project will widen 230 linear feet of an existing asphalt access road. The vehicle storage area will be approximately 20 feet wide.

SPECIFICATIONS

1. Tapers: The storage lane taper for the entrance and exit shall be 1.5:1 and 1:1, respectively.

2. Subgrade Preparation: Subgrade preparation shall be completed in accordance with Section 228 of the WV DOH Standard Specifications.

3. Pavement structure: The pavement structure shall be constructed to the thicknesses and grades provided on the attached typical pavement detail.

4. Geotextile Fabric: Engineering fabric for separation shall meet the applicable requirements of AASHTO M288, Section 7, Class 2.

5. Aggregate Base Course: The base course shall be Class 1 aggregate base course in accordance with Section 704 of the WV DOH Standard Specifications.

6. HMA Base Course: The base course shall be Marshall HMA base course, Type II in accordance with Section 401 of the WV DOH Standard Specifications.

7. HMA Wearing Course: The wearing course shall be Marshall HMA wearing course, Type I in accordance with Section 401 of the WV DOH Standard Specifications.

8. Joints: The joints between the existing and new HMA pavement shall be keyed into the existing surface for the full length of the widening project. Joints shall be constructed in accordance with Section 401.10.5 of the WV DOH Standard Specifications.

9. Lane Marking: Lane marking paint shall be alkyd resin type, ready mixed complying with AASTHO M 248 Type III. Lane markings shall be in accordance with the Manual on Uniform Traffic Control Devices for Streets and Highways, 2009 Edition, U.S. Department of Transportation.

10. Seeding and Mulching: The earthen drainage channel and all disturbed areas shall be re-vegetated with Kentucky 31 fescue, Type C, in accordance with Section 652 of the WV DOH Standard Specifications.

5.7

DELIVERY, STORAGE AND HANDLING

Equipment and materials shall be carefully handled, properly stored, and adequately protected to prevent damage before and during installation, in accordance with the manufacturer’s recommendations and as approved by the COR. Damaged or defective items shall be replaced. All items are restricted to allowed storage and staging area.

5.8

EXECUTION

1. The contractor shall contact the West Virginia One-Call System (811 or 1-800-245-4848) at least two business days prior to an excavation. The contractor shall also contact the COR at least two businesses days prior to an excavation. The COR will provide the location for underground utilities within the project area owned by the Academy.

2. Subgrade Preparation: Excavate subgrade to the required depth. The exposed subgrade should be free of surface water. When practical, proof roll subgrade with a loaded dump truck or similar equipment approved by the COR. Perform proof rolling in the presence of the COR. Undercut rutting or pumping material as directed by the COR and replace with base material. After compaction, the subgrade shall not exhibit surface deviations for pavement areas greater than ½-inch when tested with a 12-foot straightedge applied both parallel and at right angles to the centerline of the area.

3. Fabric for separation: Engineering fabric shall be installed for separation between the subgrade and base course for the full pavement area. The fabric should be relatively smooth, free of sharp protrusions, depressions, creases, or debris. The joining of fabrics shall be accomplished by sewing (either in the field by portable sewing machine or during manufacture of the fabric) and shall be installed so any splice joints have a minimum overlap of 2 feet, or as directed by the COR. Construction equipment is permitted on the fabric provided all damage is repaired in accordance with Section 715.11.3 of the WV DOH Standard Specifications.

4. Crushed Aggregate Base Course: The compacted base course shall be 12 inches thick and constructed in accordance with WV DOH Standard Specification 307. The maximum compacted lift thickness shall be 6 inches. Tailgating is not permitted. Each layer shall be thoroughly and uniformly compacted by rolling. The moisture content shall be maintained at a level sufficient to facilitate compaction. No base course shall be placed when the subgrade is frozen or when it is sufficiently wet that the surface can be marred by construction equipment.

5. HMA base and wearing courses: The compacted lift thickness of the HMA base course shall be 2.5 inches. The compacted lift thickness of the HMA wearing course shall be 1.5 inches. Immediately before placing the hot mix asphalt, clean the underlying course of dust and debris. The HMA shall be struck off in a uniform layer of such depth that, when the work is completed, it will have the required thickness and conform to the grade indicated. Regulate the speed of the paver to eliminate pulling and tearing of the asphalt mat. After placing each lift, the mixture shall be thoroughly and uniformly compacted by rolling. Compact the surface as soon as possible without causing displacement, cracking or shoving. The sequence of rolling operations and the type of rollers used shall be at the discretion of the Contractor. The speed of the roller shall, and at all times, be sufficiently slow to avoid displacement of the hot mixture and to be effective in compaction. Any displacement occurring as a result of reversing the direction of the roller, or from any other cause, shall be corrected at once. Continue rolling until the surface is of uniform texture and true to grade. To prevent adhesion of the mixture to the roller, keep the wheels properly moistened. However, excessive water will not be permitted. In areas not accessible to the roller, the mixture shall be thoroughly compacted with hand tampers. Any mixture that becomes loose and broken, mixed with dirt, contains check-cracking, or is in any way defective shall be removed full depth, replaced with fresh HMA, and immediately compacted to conform to the surrounding area. This work shall be done at the Contractor’s expense. Skin patching will not be allowed. Construction methods shall be in accordance with applicable Sections 401 and 405 of the WV DOH Standard Specifications, unless otherwise noted.

6. Joints: Key-in the existing and new HMA pavement joint for the all locations where the new pavement abuts the existing HMA pavement. The width of the key shall be 24 inches and taper from a depth of 1.5 inches at the interface of the new pavement to 0 inches within the existing pavement. The milling technique shall not cause damage to any part of the existing pavement structure that is not to be removed. A tack coat shall be applied to all joints.

7. Pavement Markings: Pavement markings consisting of a 4-inch wide solid white line shall be used to delineate the existing roadway and the widened pavement area. A 4-inch wide solid white line shall be applied for full length around the remaining perimeter of the widened pavement area. The total length of solid white line marking is approximately 475 linear feet. Pavement markings consisting of 4-inch wide broken white line shall be used to delineate the new pavement area into two 10-foot wide lanes, as directed by the COR. The broken line shall consist of 3-foot line segments and 6-foot gaps, and shall begin and terminate at the taper zones of the new pavement area. The total length of the broken white line marking is approximately 190 linear feet, including line segments and gaps.

8. Drainage: An earthen drainage channel shall be constructed a minimum of 1 foot away from the new pavement area, parallel to the lane orientation. The channel shall have 2H:1V side slopes and be a minimum of 1 foot deep. The channel shall provide drainage from the southern end of the new pavement area and continue around the perimeter of the asphalt. The depth should be increased as necessary to provide positive drainage toward the existing inlet basin located approximately 25 feet north of the project area.

9. Seeding and mulching: All disturbed areas and the earthen channel shall be seeded and mulched with Kentucky 31 fescue. Any area failing to establish a stand shall be re-seeded, re-fertilized and re-mulched as directed by the COR.

10. Disposal: Dispose of debris, rubbish, scrap, and other non-salvageable materials resulting from the paving operations with all applicable federal, state, and local regulations. Storage of waste materials on the project site is prohibited. Transport all waste materials resulting from the project from Government property for legal disposal, except waste soil. Waste soil excavated from the pavement area shall be stockpiled on the Academy property as directed by the COR.

AUTHORITY

Only the contracting officer has the authority to modify the contract, authorize changes in funding, or add additional funds for expenditure under a contract. Should the contractor question charges or instructions under this task order, the contractor will bring these issues to the contracting officer for resolution. All issues brought to the attention of the contracting officer must be presented in writing via a memorandum or e-mail.

REGULATIONS

The contractor shall perform the services as set forth in the statement of work and in accordance with all applicable laws and regulations. The contractor warrants that he will have obtained all licenses and permits required by law to engage in the activities that are required in connection with the statement of work.

WARRANTY

The contractor shall provide a 1 year warranty for material, workmanship and labor.

Unless specified otherwise, in writing and agreed upon by all parties, all products, labor, and service must be warranted by the contractor for a period not less than one year from the official date of final contractor payment.

Warranties imply that if any product or workmanship is not satisfactory during this period, it shall be replaced, adjusted, and/or repaired to the point of satisfactory, at no additional cost to the Government and is the liability of the contractor to ensure all warranties are upheld. Government will not be liable for any costs associated to travel, lodging and per diem related to warranty repairs during the specified period as stated above.

Contractor agrees to make such repairs, replacements and/or adjustment which are necessary due to settlement, shrinkage, defective workmanship and/or materials and/or installation, all in order to achieve and maintain proper operation, protection and usage.
