Instructions

Instructions
1Relevant Experience Projects demonstrate an Offeror's experience in performing under IT products contracts similar to the Government's requirement and their responsibility for successful completion during the entire BPA ordering period.
2Only one Relevant Experience Assessment spreadsheet may be submitted per Offeror or CTA team. No other format will be considered.
3Offerors are to complete the yellow highlighted sections in the "Relevant Experience" tab. At a minimum Offerors are to complete the Date, Vendor/Team Name, and Vendor/Team Name Abbreviation fill-ins.
4Project is defined as BPAs, IDIQs, Contracts, Purchase Orders, Delivery Orders.

Relevant experience is defined as past or current IT products contracts, Blanket Purchase Agreements, or orders, including Federal, State, Local and Regional governments and commercial for efforts similar to the Government’s requirement within the last three (3) years.

“Outside the Continental United States (OCONUS)” is defined as other than the 48 contiguous states, Alaska, Hawaii, and District of Columbia.

5Each Relevant Experience Project submitted must be ongoing or has been completed within the past 3 years prior to the solicitation closing date.
6Relevant Experience Project submissions must have been awarded by the U.S. Federal Government and must be:

a. A stand-alone Contract (FAR 12, 13, or 15); or,

b. A Task Order (FAR 16.505) awarded under a Single-Award Indefinite Delivery, Indefinite Quantity (SA-IDIQ) Contract or Multiple-Award Indefinite Delivery, Indefinite Quantity (MA-IDIQ) Contract (FAR 16.501-1 and 16.504); or,

c. A Task Order (FAR 8.405-2) awarded under a GSA Multiple Award Schedule (MAS); or,

d. A Task Order (FAR 8.405-2) awarded under a Single-Award Blanket Purchase Agreement (SA-BPA) or Multiple-Award Blanket Purchase Agreement (MA-BPA)(FAR 8.405-3 or FAR 13.303).

7For each Relevant Experience Factor enter Yes or No into the appropriate column in the space provided. Do not leave fill-ins blank.
8Projects in each Relevant Experience Factor may only be entered once within a specific factor however projects can be utilized again for other Relevant Experience Factors.

Example: Company A was awarded delivery order FZ8519-19-D-0001-0009 for $5,100,000 that required special asset tagging. In this example, delivery order FZ8519-19-D-0001-0009 can be used once for Relevant Experience Factor 1 for projects over $5 million and once for Relevant Experience Factor 2 for projects over $5 million that required overseas special asset tagging.

In this example, delivery order FZ8519-19-D-0001-0009 CANNOT be used multiple times for Relevant Experience Factor 1 for projects over $750 thousand, $2 million, AND $5 million and multiple times for Relevant Experience Factor 2 for projects over $750 thousand, $2 million, AND $5 million that required overseas special asset tagging. Each project in each Relevant Experience Factor number may only be entered once however projects can be utilized again for other Relevant Experience Factors.

9For Relevant Experience Factors 1, 2, 3, 4, and 9 Offerors/CTA team members are to annotate the Awardee, Date of Award, BPA, IDIQ, Contract, Purchase Order, or Delivery Order Number, Contract/Order Value, and Agency in the spaces allocated for each Relevant Experience project submitted.
Along with the submission of this Relevant Experience Assessment spreadsheet, Offerors/Teams must substantiate all Relevant Experience submitted by providing enough evidence within a BPA, IDIQ, Contract, Purchase Order, Delivery Order or Task Order document and/or other verifiable contractual documents for each Relevant Experience Project annotated. The Offeror shall highlight each section/paragraph that corresponds to the scored Relevant Experience assessment evaluation criteria they are taking credit for.
The Offeror must submit a copy of the Contract or Task Order Award Form for each Relevant Experience Project as follows:

a. Standard Form (SF) 1449 – Solicitation/Contract/Order for Commercial Items – (Block 17a identifies the Prime Contractor, Block 9 identifies the U.S. Federal Government Agency, Block 3 identifies the Award/Effective Date, and Block 31c. identifies the date the Contracting Officer signed).

b. SF 26 – Award/Contract – (Block 7 identifies the Prime Contractor, Block 5 identifies the U.S. Federal Government Agency, Block 3 identifies the Effective date, and Block 20C identifies the date the Contracting Officer signed).

c. SF 33 – Solicitation, Offer, and Award – (Block 15A identifies the Prime Contractor, Block 7 identifies the U.S. Federal Government Agency, and Block 28 identifies the date the Contracting Officer awarded/signed).

d. Department of Defense (DD) 1155 – Order for Supplies or Services (Block 9 identifies the Prime Contractor, Block 6 identifies the U.S. Federal Government Agency, Block 3 identifies the Date of Order, and Block 24 identifies the Contracting Officer signature).

e. Optional Form (OF) 307 – Contract Award (Block 7 identifies the Prime Contractor, Block 5 identifies the U.S. Federal Government Agency, Block 2 identifies the Effective date, and Block 15C identifies the date the Contracting Officer signed).

f. GSA Form 300 – Order for Supplies and Services (Block 6 identifies the Prime Contractor, Block 10 identifies the U.S. Federal Government Agency, Block 1 identifies the Date of Order, and Block 26C identifies the date the Contracting Officer signed).

g. Other Official Government Award Form from a Cognizant U.S. Federal Government Agency. (Must explicitly identify the Prime Contractor, U.S. Federal Government Agency, the Effective Date, and the date the Contracting Officer signed).

h. Other Official U.S. State Government, International Public Sector, and/or Non-U.S. Federal Government/Commercial Award Form. (Must explicitly identify the Prime Contractor, Awarding Entity, the Effective Date, and the date the Authorized Representative signed).

Evidence of successful completion of each contract or order shall be submitted in the form of a paid invoice.

10 The ability to handle a large number of orders is essential for the success of the of this acquisition. For Relevant Experience Factor 5 Offerors/CTA team members are to annotate their top five BPAs or Multiple Award Contracts with the highest number of orders against them within the past three years. Offerors are to annotate the highest number of orders received on each of the BPAs or Multiple award contracts received annually within the past three years. Only one BPA or Multiple Award Contract can be listed per CTA team member.

For each BPA or Multiple Award Contract number reported, Offerors are to note the awardee/CTA team member, total number of orders received annually (highest annually within the past 3 years), annual period for the stated quantity (i.e. FY18, 2017, etc.), Government point of contact, Government point of contact phone number, and Government point of contact email address.

For each BPA or Multiple Award Contract Number reported in Relevant Experience Factor 5, Offerors are to provide a listing of all the orders that correspond to the BPA or Multiple Award Contract Number they are taking credit for to validate the information provided. The following information is to be specified for each order:

Order Number Date Dollar Value Summary of what was purchased (i.e. computer monitors, routers, laptops, etc.)

10For Relevant Experience Factor 6 Offerors/CTA team members are to annotate the BPAs or Multiple Award Contract numbers, award date, and team members for the award specified in the space allocated for each Relevant Experience project submitted. The Offeror shall submit documented evidence of the teaming agreement and evidence of performance of each team member that directly corresponds to the contract or order number of the Relevant Experience project. See Instruction 13 for labeling instructions.

11 For Relevant Experience Factor 7 Offerors/CTA team members are to annotate the number of specific CMMI and ISO certifications per team in the spaces allocated. Offerors/CTA team members are to provide evidence of certifications. Regardless of the point value awarded as shown in factor 7.a. through 7.e the maximum point value for factor 7 in its entirety is 300 points.

All Certifications must be in the name of the Official Legal Entity of the Offeror unless a Meaningful Relationship Commitment Letter is provided. All certifications must be currently in place at the time of the solicitation’s closing date. Certifications must be maintained and the current level at time of award or higher throughout the 2GIT BPA period of performance.

12 For Relevant Experience Factors 8 Offerors/CTA team members are to provide evidence of Government Facility Clearances.

All Clearances must be in the name of the Official Legal Entity of the Offeror unless a Meaningful Relationship Commitment Letter is provided. All Clearances must be currently in place at the time of the solicitation’s closing date. Clearances must be maintained and the current level at time of award or higher throughout the 2GIT BPA period of performance.
