PAST PERFORMANCE QUESTIONNAIRE

Please complete and submit this reference directly to the GSA Contracting Specialist (roy.hardy@gsa.com) and the GSA Contracting Officer (melissa.martel@gsa.com) no later than Friday, April 23, 2021 at 3:00 PM ET.

PROJECT: Quarterly Linearity Testing & Preventive Maintenance for the CEMS and Opacity Monitors.

Past performance based on historic records and work performed by contractor, knowledge of the GSA system, reports submission, response to issues and performance of all tasks included in scope of work.

This evaluation considers the quality of the offeror’s past performance on similar projects, including (but not limited to) quality of service, timeliness, and technical performance.

Section 2 – To be completed by the Reference Please complete the following questionnaire on behalf of the contractor listed in Section 1. Only the project listed shall be considered for this evaluation. For each question below, select the rating that best describes the Contractor’s performance. Please provide a narrative explanation where appropriate; any narrative comments are greatly appreciated. If the contractor has completed other work for your agency/organization in the last five (5) years, you may note that as additional comments.

The completed questionnaire should be emailed directly to the Contracting Office to ensure the confidentially of your responses. Your responses will be reviewed solely for the purposes of a past performance evaluation and no one other than the selection committee will see your response.
