Instructions for items other than those that are self-explanatory, are as follows:

Item 1 (Contract ID Code). Insert the contract type identification code that appears in the title block of the contract being modified.

For a solicitation amendment, change order, or administrative change, the effective date shall be the issue date of the amendment, change order, or administrative change.

For a supplemental agreement, the effective date shall be the date agreed to by the contracting parties.

For a modification issued as an initial or confirming notice of termination for the convenience of the Government, the effective date and the modification number of the confirming notice shall be the same as the effective date and modification number of the initial notice.

For a modification converting a termination for default to a termination for the convenience of the Government, the effective date shall be the same as the effective date of the termination for default.

For a modification confirming the contracting officer's determination of the amount due in settlement of a contract termination, the effective date shall be the same as the effective date of the initial decision.

Item 14 (Description of Amendment/Modification).

Organize amendments or modifications under the appropriate Uniform Contract Format (UCF) section headings from the applicable solicitation or contract. The UCF table of contents, however, shall not be set forth in this document.

Indicate the impact of the modification on the overall total contract price by inserting one of the following entries:

1. Please confirm that an official teaming agreement document between the design firm and the general contractor will be required with the proposal.

Section II.E.Requirements of Joint Venture Offerors details the submissions for a Joint Venture.

Any other contracting arrangements do not need to be provided.

2. Solicitation Section II.B. – Please confirm cover pages and tab pages will be excluded from the 35-page limit.

Those documents are included in the page count.

3. Please confirm whether the Representations and Certifications documents will be counted towards the page count.

They do not.

4. Please confirm the font, margin, and spacing requirements of the proposal format.

There are no minimum or maximum requirements.

5. Solicitation Section II.C. Factor 4 – Please confirm that completed CPARS will be accepted in lieu of completed PPQs for the submitted projects.

Yes, this is acceptable. Please confirm the Point of Contact is still accurate in the CPARS.

6. Factor 1 Submittal Requirements – the solicitation states that the submitted projects will be considered similar if the contract value at award is in excess of $10,000,000. Due to the number of elevators/escalators within most facilities, it is very rare that elevator/escalator specific modernization projects are awarded at excess of $10,000,000. Please consider lowering the contract value threshold requirement to $2,800,000 as design-build elevator modernizations over $2,800,000 are less frequent.

The threshold is lowered to $7M.

7. Factor 2: Approach to Design-Build. Please consider extending the 3-page limit for the written narrative to 8 pages to allow for our narrative to adequately meet the solicitation requirements for Factor 2.

The page limit remains unchanged at 3.

8. If available, please provide the Past Performance Questionnaire in Microsoft Word format.

9. We have existing completed PPQs for our selected projects. Can these completed PPQs be submitted with our proposal in lieu of having our clients complete new PPQs for this proposal?
