For a solicitation amendment, change order, or administrative change, the effective date shall be the issue date of the amendment, change order, or administrative change.
For a supplemental agreement, the effective date shall be the date agreed to by the contracting parties.
For a modification issued as an initial or confirming notice of termination for the convenience of the Government, the effective date and the modification number of the confirming notice shall be the same as the effective date and modification number of the initial notice.
For a modification converting a termination for default to a termination for the convenience of the Government, the effective date shall be the same as the effective date of the termination for default.

Item 14 (Description of Amendment/Modification).
Organize amendments or modifications under the appropriate Uniform Contract Format (UCF) section headings from the applicable solicitation or contract. The UCF table of contents, however, shall not be set forth in this document.
Indicate the impact of the modification on the overall total contract price by inserting one of the following entries:

In addition to the questions and answers below the RFI cutoff date is 12:00 pm CST on August 2, Q1: Should offerors submit separate proposal volumes for 1) price, 2) past performance, and 3) technical?
Or should offerors submit one proposal volume?
A1: There is no required structure. Please be sure all documents are clearly labelled.
Q2: Is there a page limit to the size of the proposal? If so, please specify.
A.2: No.
Q3: Is there an electronic file size limit for the proposal, e.g., 10 megabytes?
A3: GSA email accounts can receive file sizes up to 20 MB.
Q4: Section M.1 says the Government will make a “best value” award decision. However, the RFP does not specify the specific proposal evaluation factors and the relative order of priority of the proposal evaluation factors. Can the Government specify the relative priority of proposal evaluation factors to include: price, quality control program, past performance, etc.? The current Section M language for proposal evaluation is ambiguous and it is unclear what evaluation factors the Government will use to make decisions.
A4: Reference Section M.1 Basis of Award. The evaluation factors to be used are total evaluated price and past performance. Past performance is significantly more important than price.
Q5: RFP Section L.5. Inconsistencies in Specifications states: “ Offerors are prohibited from modifying, in any way, shape, or form, any documents, printed or electronic, associated with this solicitation, including, but not limited to, the specifications and any amendment(s) thereto .” Given the scope and complexity of the Region 5 requirements, there are many nuances to the price structure. If an offeror submits a list of price assumptions with its offeror, is this considered to be non-responsive to RFP Section L.5? Will submitting price proposal assumptions result in a rejection of the proposal?
A5: Proposal pricing should be based on the requirements in Section B. GSA cannot accept proposals with added clarifications that go beyond what is in the scope of work. Any assumptions should be clarified during the RFI period.
Q6: Can offerors submit supplement information with the “Attachment A” form, e.g., work site photographs, client testimonials, additional work descriptions, etc. Can offerors provide a lead-in narrative describing the past performance references provided?
A6: That information may be provided, however, GSA will only use responses from the references provided on Attachment A to evaluate the prospective offeror.
Q7: If an offer has a GSA Schedule contract, must it submit these rates?
A7: No.
Q8: Given the scope and complexity of the Region 5 Moving Services RFP, will the Government consider conducting a pre-proposal conference?
A8: GSA does not plan to hold a pre-proposal conference.
Q9: Can the Government provide expected order quantities and dollar volumes for the Region 5 Moving Services requirement?
A9: See sections F.8 Guaranteed Contract Minimum and F.9 Maximum Total Contract Amount.
Q10: Can the Government list all the incumbent contractors, their DUNS numbers, and obligated dollar amounts on each of the current contracts?

The current maximum total contract amount is $6,000,000.00 per award per zone for services during the one year base period of the contract, and $2,000,000.00 for each option year.
Q11: Will there be union pricing in specified markets within this contract, if so please provide those specific markets.

Q12: Since our nation and its economy was essentially shut down over the last two years, can we submit contract/references/recent jobs that completed within seven years from closing of the solicitation?
A12: No.
Q13: The solicitation states that offerors shall submit GSA Form 3501 and 3502; however, the GSA website Forms search states these forms are obsolete with now replacement. Should the offeror omit?
A13: Please disregard the reference to forms 3501 and 3502. See Exhibit 2, FAR 52.209-7 and Section I Contract Clauses- the Clauses Incorporated by Reference table for 52.209-9.
Q14: Can offeror submit electronic proposal version and/or paper version?
A14: GSA will only accept electronic proposals for this solicitation.
