About this file

This document is a Government-Wide Debarment and Suspension (Nonprocurement) certification form involving the U.S. Department of Transportation (U.S. DOT) and the U.S. Office of Management and Budget (U.S. OMB). The form requires contractors, subcontractors, and recipients participating in covered transactions to verify their eligibility to engage in federal, state, and local public transactions by certifying they are not presently debarred, suspended, proposed for debarment, declared ineligible, voluntarily excluded, or disqualified.

The certification requires participants to confirm that within the preceding three-year period, their management has not been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or other specified misconduct. Participants must also verify they are not currently indicted or criminally charged, have not had public transactions terminated for cause, and will promptly report any contradictory information. The document mandates that lower-tier contracts exceeding $25,000 or requiring federal official consent must comply with federal debarment and suspension regulations, ensuring each participant in a federally funded project is not prohibited from participation.

GOVERNMENTWIDE DEBARMENT AND SUSPENSION

(NONPROCUREMENT)

Recipients, contractors, and subcontractors that enter into covered transactions are required to verify that the entity (as well as its principals and affiliates) with which they propose to contract or subcontract is not excluded or disqualified.
