About this file
This document is a Vendor's Statement form from the City of Owensboro, Kentucky, pursuant to Kentucky Revised Statutes (KRS) 45A.343, which requires vendors to disclose and comply with specific state tax and labor regulations. The form serves as a legal declaration by a vendor seeking to enter into a contract with the City of Owensboro, certifying their compliance with various state statutory requirements related to corporate taxes, sales and use taxes, income taxes, wage and hour laws, occupational safety, unemployment, and workers' compensation.
The statement mandates that the vendor affirm they have not been finally determined to have violated any of the specified KRS chapters within the past five years and commits to ongoing compliance throughout the contract period. Failure to reveal any final violations or maintain compliance could result in contract cancellation and a two-year disqualification from future City of Owensboro contract opportunities. The document is designed to be completed and signed by an authorized company representative, with spaces provided for the company name, signatory details, and date of execution in 2025.
