About this file
This document is an Exhibit "C" detailing insurance requirements for a contract between a Provider and Forsyth County Board of Commissioners in Cumming, GA. The exhibit outlines comprehensive insurance coverage specifications that the Provider must maintain throughout the agreement, including six distinct types of insurance with specific minimum coverage limits. These insurance types encompass Workers' Compensation, General Liability, Automobile Liability, Excess Umbrella Liability, Professional Liability/Medical Malpractice, and Cyber Liability/HIPAA Coverage.
The insurance requirements mandate precise documentation and notification protocols, such as providing a certificate of insurance with an additional insured endorsement, maintaining an extended reporting period of at least five years, and delivering renewal policies at least 30 days prior to policy expiration. The specified minimum coverage limits range from $500,000 for Cyber Liability to $3,000,000 for Excess Umbrella Liability, with the Provider required to send all insurance documents directly from the insurance company to the County representative. If the Provider maintains higher limits than specified, the County is entitled to coverage for any additional insurance proceeds exceeding the stated limits.
