About this file

This document is a benefits and services schedule for the State of Florida Retiree Classic (HMO) Medicare health insurance plan offered by Capital Health Plan. The schedule outlines comprehensive medical coverage for Medicare-eligible Florida state retirees, detailing copayment amounts for various healthcare services including physician visits, hospital stays, emergency services, preventive care, mental health treatments, diagnostic procedures, and prescription drugs. The plan provides in-network coverage with fixed copayments ranging from $0 to $150 per service, with an annual maximum out-of-pocket medical expense limit of $2,500 per member and a prescription drug out-of-pocket maximum of $2,000.

The plan features a tiered prescription drug pricing structure with copayments varying by drug tier (generic to specialty), and offers additional benefits such as home health services, hospice care, skilled nursing facility coverage, and routine dental and vision care. Prescription drugs are categorized into six tiers with copayments ranging from $0 to $95 for 30-day supplies, depending on the medication type. The plan also includes exclusions for services not specifically listed in the Evidence of Coverage, such as hearing aids, cosmetic surgery, and non-prescription drugs, with members responsible for charges exceeding maximum benefit limitations.
