About this file

This document is a Debarment and Suspension Certification form from the City of San Angelo, Texas Purchasing Division, located at 72 West College Avenue. The certification requires prospective primary vendors to attest to their legal and ethical standing when bidding on city contracts.

The certification mandates that vendors declare they: (a) are not currently debarred or suspended from Federal transactions, (b) have not been convicted of fraud or criminal offenses in public transactions within the past three years, (c) are not currently indicted for governmental offenses, and (d) have not had public transactions terminated for cause within the past three years. If a vendor cannot certify these statements, they must attach an explanation to their bid. The form provides space for an authorized agent to print their name, title, and provide a signature and date of certification.
