About this file
This is a Scope of Work document for the Healthy Utah Community Designation Program, a contract between the Utah Department of Health and Human Services (DHHS) and a contractor for program management and implementation during fiscal year 2027 (July 1, 2026 through June 30, 2027). The contractor is responsible for overseeing the complete management and implementation of the designation program, which recognizes Utah cities and towns implementing evidence-based health improvement strategies. Primary deliverables include maintaining a website to host the application portal and showcase designated communities, providing technical assistance to municipalities applying for designation, and recognizing qualified communities at the Utah League of Cities and Towns mid-year and annual conferences. The contract term is for one fiscal year with performance measured by tracking the number of applications received, communities designated, and health strategies implemented during the contract period.
This is a cost reimbursement contract with a maximum contract value of $250,000.00, allocated entirely to program management and implementation expenses. The contractor must submit monthly invoices to the Healthy Environments Active Living Program no later than the 15th day of the month following the activity month, with all expenditures directly related to contract performance eligible for reimbursement. Progress reporting is required quarterly through submission of reports via Qualtrics. The Notice of Intent to Award indicates that DHHS intends to award this contract without engaging in standard competitive procurement, claiming that only one source exists for this procurement item; the justification documentation is referenced in the Files section but specific reasoning is not detailed in this Scope of Work document.
