About this file

This document is an Acknowledgements & Preference Application form for a Louisville Metro Government contract opportunity. The form requires vendors to acknowledge receipt of any addenda to the proposal and provides options for certifying minimum wage and local vendor preferences. Vendors can indicate whether they pay all full-time employees at least $10.10 per hour and whether they meet the local vendor requirements as described in Section I of the proposal.

The document outlines potential penalties for false certification, including a liability of 30% of the contract amount if minimum wage information is falsely provided, and a fine of 25% of the total contract if local vendor preference is granted based on false information. The form includes check boxes for vendors to affirm or deny their eligibility for minimum wage and local vendor preferences, referencing specific Louisville Metro Code (LMCO) sections §37.56 and §37.03 respectively.
