About this file

This is a Sole Source Justification document for a federal contract award using Simplified Acquisition Procedures. The U.S. Department of Homeland Security's U.S. Citizenship and Immigration Services (USCIS) Office of Contracting proposes to award a Purchase Order to Dentrust Dental International, Inc., doing business as DOCS Health (SAM Unique Entity ID: YWF6YDZ6GLL5), located in Pipersville, Pennsylvania, for medical support services. The contract covers pre-employment and post-employment medical testing, independent medical examinations, reviews of medical documents, and post-employment psychological testing services for USCIS Law Enforcement personnel in accordance with U.S. Department of Health and Human Services regulations. Services will be delivered across all 50 U.S. states and U.S. territories including Puerto Rico, the Virgin Islands, and the Mariana Islands. The Independent Government Cost Estimate (IGCE) is $344,250.00 for one twelve-month base period with a Period of Performance from February 2, 2026, to February 1, 2027.

The procurement is designated as a sole source award under FAR 13.106-1(b)(1) because only one responsible source is capable of providing the specialized services required. The awardee currently holds an active DHS Immigration and Customs Enforcement (ICE) contract (number 70CMSD24C00000001) for identical services and is performing comparable requirements with satisfactory results. Price reasonableness will be determined through comparison with prices previously paid under the existing DHS ICE contract for the same or similar services. The Government will be in daily contact with the Contractor due to the complex and technical nature of the requirements, with requests for pre-employment testing originating from the USCIS Office of Human Capital and Training Human Resources Operations Center and employee examinations originating from the USCIS Fitness for Duty Program. The Contracting Officer, Joshua S. Riley, approved the justification, and USCIS intends to comply with public display and synopsis requirements under FAR 13.105(a).

Sole Source (including brand name) Justification, using Simplified Acquisition Procedures, Exceeding the Micro-purchase Threshold and up to the Simplified Acquisition Threshold, Medical Support Services

REFERENCE: Justification Log Number: FY26-0044 and PR Number: SEC260018

1. Agency and Contracting Activity.

The Department of Homeland Security, U.S. Citizenship and Immigration Services (USCIS), Office of Contracting, proposes to award a Purchase Order, on behalf of the Office of Security & Integrity.

2. Nature and/or description of the item/service being procured and anticipated cost.

This requirement encompasses pre-employment and post-employment medical and post-employment psychological testing services for USCIS Law Enforcement personnel. Pre-employment medical services include selectee medical and psychological testing in accordance with U.S. Department of Health and Human Services (HHS) regulations. Post-employment medical services include independent medical examinations, reviews of medical documents, and psychological testing for current USCIS employees.

Potential locations where services will be delivered include all 50 U.S. states, plus locations in Puerto Rico, the Virgin Islands, and the Mariana Islands to include Guam. Requests for pre-employment testing will originate from the USCIS Office of Human Capital and Training (HCT) Human Resources Operations Center (HROC). For USCIS employees, examinations and testing will originate from the USCIS Fitness for Duty Program (FFDP).

Due to the complex and technical nature of the requirements, the Government shall be in contact with the Contractor daily. The Contractor shall be prepared to be responsive to inquiries and status requests.

The circumstances necessitate a sole source procurement because only one responsible source is capable of providing pre-employment and post-employment medical and post-employment psychological testing services that fully meet the unique requirements of USCIS law enforcement personnel. These services require specialized expertise in federal law enforcement medical and psychological standards, the ability to conduct secure and compliant evaluations, and demonstrated experience supporting DHS operational and security requirements. Based on an evaluation of the contractor’s documented capabilities and prior performance, the identified source provides comprehensive federal occupational health services, including pre-employment and employee medical examinations, fitness-for-duty evaluations, independent medical examinations, functional capacity assessments, drug testing and Medical Review Officer services, physical fitness testing, reviewing physician support, and case management. This breadth of services directly corresponds to the USCIS requirement and enables the contractor to perform the full scope of medical and psychological testing without reliance on multiple subcontractors, thereby reducing operational

Sole Source (including brand name) Justification, using SAP, Exceeding the MPT and up to the SAT, Medical Support Services Justification Log Number: FY26-0044 risk. Market research indicates that no other vendor is capable of meeting all technical, regulatory, and schedule requirements within the required timeframe without unacceptable impact to mission readiness. Accordingly, award to a single source is necessary to ensure continuity of services, compliance with applicable standards, and the medical and operational fitness of USCIS law enforcement personnel.

The Government Independent Government Cost Estimate (IGCE) for this effort is estimated at:

$344,250.00 for one (1) twelve (12) month base period with an anticipated Period of Performance of 02/02/2026 – 02/01/2027.

Proposed Awardee:

Dentrust Dental International, Inc. Doing Business As (DBA) DOCS Health 6097 Easton RD Pipersville, Pennsylvania 18947-1810, United States SAM Unique Entity ID: YWF6YDZ6GLL5

3. Identification of the authority and rationale.

The proposed procurement will utilize simplified acquisition procedures and will be awarded on a sole source basis in accordance with FAR 13.106-1(b)(1) and as specified below.

Only one source reasonably available:

☒ Other. Only one source is capable.

Rationale:

USCIS’s rationale for this sole source determination is based on the highly specialized nature of federal law enforcement pre-employment and post-employment medical and post-employment psychological testing services required to support its mission. In accordance with FAR 13.106-1(b)(1)(i), the Contracting Officer has determined that only one source is reasonably available that can meet the Government’s requirements. These services must be performed in compliance with DHS and federal law enforcement standards and require demonstrated experience conducting secure, compliant evaluations for law enforcement personnel. The selected awardee currently holds an active DHS ICE contract for identical services under contract number 70CMSD24C00000001 and is performing requirements of comparable size, scope, and complexity with satisfactory results. This established performance demonstrates the contractor’s ability to meet DHS-specific technical, security, and quality requirements without disruption. Utilizing this contractor ensures continuity of services, consistency in evaluation methodologies, and compliance with applicable standards across DHS law enforcement components. Award to any source other than the identified contractor would present an unacceptable risk to mission execution, including delays in onboarding and retaining law enforcement personnel, potential inconsistencies in medical and psychological determinations, and adverse impacts to USCIS law enforcement workforce readiness.

Accordingly, the Contracting Officer determines that this requirement shall be solicited from

Sole Source (including brand name) Justification, using SAP, Exceeding the MPT and up to the SAT, Medical Support Services Justification Log Number: FY26-0044 and awarded to the identified single source as the only source reasonably available to satisfy the Government’s needs.

4. The anticipated price will be determined fair and reasonable based on one or more of the price analysis techniques described below in accordance with FAR Part 13.106-3(a)(2).

☐ Market Research;

☒ Comparison of the proposed price with prices found reasonable on previous purchases;

☐ Current price list;

☐ Comparison with similar items in a related industry;

☐ Comparison to Independent Government estimate;

☐ Contracting Officer’s personal knowledge; or ☐ Other reasonable basis.

Price reasonableness will be determined in accordance with FAR 13.106-3(a)(2) through a comparison of the proposed price to prices previously paid for the same or similar services under prior Government contracts. Specifically, the Contracting Officer will compare the proposed pricing determined fair and reasonable under DHS ICE contract number 70CMSD24C00000001, which covers identical pre-employment and post-employment medical and post-employment psychological testing services of similar size, scope and complexity. This comparison will provide a reasonable basis to conclude that the proposed price is fair and reasonable, as the prior ICE pricing was established through appropriate Government review and accepted as reasonable for comparable requirements.

5. Other Information.

USCIS intends to comply with the public display and synopsis requirements in FAR 13.105(a).

APPROVAL:

Joshua S. Riley Contracting Officer USCIS, Office of Contracting
