About this file
This is a Request for Statement of Qualifications (SOQ) issued by the State of Utah Division of Facilities Construction and Management (DFCM) to prequalify code inspection firms for FY2027. The solicitation seeks to establish a prequalified list of contractors to provide code and special inspection services for statewide capital projects throughout Utah. A mandatory meeting is required on May 13, 2026, at 1:00 PM at the Taylorsville State Office Building for all contractors seeking prequalification. Questions must be submitted by May 20, 2026, at 3:00 PM, with addenda due May 27, 2026, at 3:00 PM. SOQ proposals are due June 3, 2026, at 12:00 NOON via the Utah Public Procurement Place (U3P) portal. Prequalified contractors will be announced on June 24, 2026, at 5:00 PM, and prequalified status will be effective July 1, 2026, and expire December 31, 2027, for an 18-month period. Contractors not attending the mandatory meeting are ineligible for prequalification.
The prequalification process utilizes a selection committee composed of DFCM and state agency employees who will evaluate proposals using a 100-point scoring system: five points for pass/fail documentation requirements (Code Inspector Information Form and Termination/Debarment Certification Form), 70 points for Statement of Qualifications and Management Plan, and 25 points for Past Performance References. Contractors must achieve a minimum overall score of 80 points and maintain an average past performance rating of at least 4.00 out of 5.00 to be prequalified. Contractors without three or more DFCM performance ratings must provide a minimum of five non-DFCM references from projects completed within the past five years. The SOQ proposal must not exceed eight pages and must include organizational charts, inspector resumes with ICC certifications and State of Utah licenses, and a management plan addressing work assignment, prioritization, and adherence to DFCM policies. Contractors who do not self-perform special inspections and testing services must subcontract with DFCM-approved special inspection firms. Prequalified status is renewable for successive 18-month periods, with contractors required to submit renewal documents within 60 days prior to expiration.

DFCM Stage I Request for Statement of Qualifications for Prequalified Code Inspection Firms, 4.21.2026 2
Solicitation Summary The State of Utah Division of Facilities Construction and Management (DFCM) is issuing this Open-ended Stage I Request for Statement of Qualifications solicitation to prequalify new code inspection firms (“Contractors”) to perform code and special inspection services for statewide capital projects throughout the State of Utah.
The prequalification of Contractors is based on the prequalification process and selection criteria provided in the solicitation documents.
All Contractors interested in becoming prequalified with DFCM must meet the minimum requirements based on the prequalification process and selection criteria provided in the solicitation documents. Proposing Contractors who do not meet these requirements will be considered non-responsive and will be ineligible for prequalification at this time.
What is the difference between a Stage I Request for Statement of Qualifications solicitation and a Stage II solicitation?
- The Stage I solicitation process is the process DFCM uses to evaluate Contractor qualifications and experience, and determine whether or not they are qualified to provide code inspection services for DFCM capital projects.
This process utilizes a selection committee made up of three or more DFCM and state agency employees to perform an evaluation of each contractor’s qualifications and experience. The Stage I process is commonly referred to as the “prequalification” process.
- Stage II solicitations are used to select Contractors for individual DFCM capital projects. In order for a Contractor to be considered for the award of a Professional Services Contract for Code and Special Inspection Services, they must be a Stage I prequalified Contractor listed on the “Prequalified Code Inspection Firm” list on the DFCM website. Contracts for individual projects may be issued via direct award or through a Stage II solicitation process which are issued on a per-project basis and posted in U3P.

Non-DFCM Performance References If a Contractor does not have a minimum of three active DFCM performance ratings, they are required to submit references for non-DFCM contracts completed within the past five years that will supplement any of the Contractor’s DFCM performance ratings. All references must be for projects in which the Contractor was the prime Contractor.

Incomplete reference information or the inability for DFCM to acquire a rating from a provided reference may result in the Contractor being disqualification from the selection process.

To obtain a minimum of three collective performance ratings for each Contractor, references provided for non- DFCM projects will be contacted at random and asked a series of predetermined questions. The reference will be asked to rate the Contractor on a scale of 1 (low) to 5 (high) for each question. The ratings will be averaged and result in a performance rating for the corresponding project. DFCM will only contact references until a collective total of three performance ratings for non-DFCM and DFCM projects are obtained. The three performance ratings will be averaged and used in the selection scoring matrix. Contractors that fail to meet this requirement or a minimum of three collective ratings may be disqualified from the selection process.
Contractors must receive an average performance rating of 4.00 out of a possible 5.00 to prequalify as a DFCM Prequalified Contractor. Contractors receiving an average rating of less than 4.00 will not be prequalified regardless of how they score on other selection criteria areas.
It is the Contractor’s responsibility to ensure up-to-date contact information is provided for their references. If DFCM is unable to obtain the minimum number of references required, DFCM reserves the right to reject the Contractor’s SOQ proposal for this round of prequalification.
