About this file
This document is a Truth in Negotiation Certification form from the State of Florida Department of Transportation (FDOT), specifically form 375-030-30 dated 05/14, which is required for professional services contracts exceeding the threshold amount specified in Section 287.017 for Category Four. The form mandates that consultants provide a certification of accuracy regarding wage rates and factual unit costs supporting the compensation for a project agreement.
The certification requires the consultant to warrant that the wage rates and unit costs are accurate, complete, and current at the time of contracting. The document stipulates that if the Department determines the agreement price was increased due to inaccurate, incomplete, or noncurrent wage rates, the contract price will be adjusted accordingly. Such adjustments must be made within one year following the contract's end, which is defined as the date of final billing or work acceptance by the Department, whichever occurs later. The form provides space for the consultant's name, signature, and date of certification.
