C. Obtain Infection Control permit from COR. Do not begin work until the following infection control measures have been installed , inspected by the COR, and approved.

1. Securely seal and block off supply air diffusers and return air grills. Seal shall be dust-tight and withstand airflow.

2. Construct infection control barrier to seal work area from non-work areas. Barrier shall include an Anteroom that allow workers to remove protective clothing or vacuum off existing clothing and clean tools when exiting the work area. Barriers can be made from:

a. Closed door with masking tape applied over the frame and door. This is acceptable for work that can be contained in a single room; however, maintenance of negative air pressure in separate rooms may be difficult for Contractor to achieve.

b. Airtight plastic barrier that extends from the floor to ceiling. Seams shall be sealed with duct tape to prevent dust and debris from escaping. Use fire retardant polystyrene, 6-mil thickness. Supporting structure shall be non-flammable. Wood products, even treated with fire retardant, are not allowed.

c. Steel-framed drywall barrier, with sealed joints. (If the fire system will be modified or hot work inside the work area is anticipated, this barrier can be converted to a 1-hour smoke barrier.)

3. Seal holes, pipes, conduits, and all other penetrations through the continuous barrier.

4. Maintain negative air pressure (0.1 inches of water gauge) within work site. Use High Efficiency Particulate Air-Equipped filtration units to maintain negative air pressure. The HEPA units shall be rated to at least 95% capture of 0.3 microns including pollen, mold spores, and dust particles. HEPA filters shall have ASHRAE 85 or other prefilter to extend the useful life of the HEPA. Provide both primary and secondary filtrations units. Maintenance of equipment and replacement of the HEPA filters and other filters shall be in accordance with manufacturer’s instructions.

5. The HEPA negative pressure machines shall discharge outside the hospital (this will probably have to be done through a window fitting). Exhaust hoses are to be heavy duty, flexible steel reinforced ventilation blower hose. Exhaust shall be placed so that dust is not reintroduced to the medical center – at least 20 feet from doorways and air intakes.

6. Place manometers with data logging capability with the sensor inside the work barrier but reading outside the ante room.

7. For the work area where exterior venting cannot be made, a HEPA air scrubber may be used rather than a negative air machine. The scrubber shall provide a minimum of 6 air changes per hour; the volume of space indicated on the drawings contains _____ cubic feet, so the scrubber shall have a _____cubic feet per minute (cfm) capacity.

8. Before any construction on site begins, all contractor personnel involved in the construction or renovation activity shall be educated and trained in infection prevention measures established by the medical center. This shall take place in the ________ meeting described in paragraph _________ above.

D. Daily operations

1. Workers shall have dust-free clothing when exiting the work area, including shoes, hard hats, safety vests, and gloves. The ante-room shall have sufficient space for workers to vacuum their clothing and to wipe hard-surfaced materials.

2. Alternatively, workers may wear “bunny suits” in the work area, which are donned to enter the work area, and removed in the ante room before exiting the work area. Contractor shall provide facilities for storing protective clothing off the floor (hooks or clothing rack). If shoe covers cannot be worn because of safety concerns, boots shall be cleaned after exiting the work area.

3. Workers shall place tool belts, rags, tool belts, and small tools inside clean portable containers when being transported through the hospital, and not wear them on their person.

4. Equipment, materials, waste carts, and transfer carts shall be dust-free when outside the work area. Workers shall wipe the exposed exterior down in the ante room before exiting the work area and also before bringing them into the hospital from the outside.

5. Contractor to provide a HEPA filtered vacuum cleaner in the ante room to allow cleaning of clothing, tools, and carts as they exit the work area.

6. Contractor to provide a separate HEPA filtered vacuum cleaner in the work area to clean the workspace of dust and fine particles. Larger debris may be swept up, but dust suppressant shall be used to prevent dust from being stirred into the air.

7. Power tools such as drills and saws shall have a working-surface shroud with HEPA-vacuum attachment.

8. Contractor to provide commercial disposable disinfecting wet wipes in the ante room to allow cleaning of clothing, tools, and carts as they exist the work area, and at the location tools, materials, and carts enter the hospital from the outside. Contractor to provide disposal bins lined with plastic bags in these areas for used wipes, of a size that shall contain the amount used in a day. Empty wipe-bin throughout the day and at the end of each work day.

9. Contractor to place adhesive walk-off mat within ante room at the exit to the adjacent corridor, and outside the anteroom door. Workers shall pull sheet off tacky mats and dispose of frequently to maintain a dust-free area directly outside the ante room at all times.

10. Workers to remove debris as it is created. Debris shall be hauled in enclosed dust proof containers or wrapped in plastic and sealed with duct tape. No sharp objects shall be allowed to cut through the plastic. Workers to wipe down the exterior of the containers with a damp rag to remove dust. Solid covers shall be kept clean and stored on shelves or racks (not on the floor) when not in use, preferably in a bag.

11. If workers displace a ceiling tile for visual inspection, they shall replace it immediately. They shall immediately vacuum up particles that were disturbed by the displacement. If the ceiling shall remain open for more than 10 minutes, Contractor shall install dust control barrier above ceiling to contain the work.

12. There shall be no standing water during construction. This includes water in equipment drip pans and open containers within the construction areas. Workers shall clean up all accidental spills so that they are dry within 12 hours. Contractor to remove and dispose of porous materials that remain damp for more than 72 hours.

13. Contractor shall clean work area at the end of every work day at a minimum. This includes removing debris, vacuuming small particulate matter with HEPA filtered vacuum, and damp mopping finished floors. If dust is being tracked out work area, it shall be cleaned more often to ensure dust is not tracked out of work area.

14. Contractor shall clean HEPA filtered vacuums inside and out at the end of every work day. Conduct detailed daily inspection of the vacuum to ensure clean, intact filters and proper seating of the HEPA filter on the filter gasket.

15. Contractor shall conduct detailed daily inspection of the HEPA filtered negative air machine to ensure clean, intact filters and proper seating of the HEPA filter on the filter gasket.

E. At the completion of work, the Contractor shall dispose of all remaining waste, remove tools and remaining materials, and thoroughly clean the work area.

1. Cleaning shall be by HEPA vacuum followed by commercially available disposable wipes and mops. Clean all vertical and horizontal surfaces. Request an inspection of the area by the COR.

2. Wipe down the barriers with disinfecting wipe prior to moving or taking them down to minimize the spread of dust on the barriers.

3. When the cleanliness of the area has been approved by the COR, remove air supply and air return blocks and seals. Carefully remove barrier materials to minimize creation of dust and debris. Immediately clean up any dust and debris created with HEPA vacuum and disposable wipes.

4. Sign off on permit and return to COR.

5. After the barrier has been removed, the COR will coordinate an Environmental Services terminal clean of the area to the level required for the type of patient care that will be provided in the area.
