Patient Centric Labels BPA – CMOP Lancaster and CMOP Leavenworth Amendment 1
1. DISREGARD - Page 6 last bullet point - (this is a Permanent adhesive spec and NOT a Temporary Adhesive spec)
· A “Peel Adhesion Test” for the PC label temporary adhesive (slow setting glue) should be between 1.15 and 1.45 with a target of 1.3
· Below this range will cause the label to come off too easily
· Above this range will cause the label to be too adherent.
· Glue sets up in 24 hrs
2. The Temporary Adhesive spec page 12 is still applicable Again, this is an estimated adhesive spec – may require adjustment after award.
Estimated Spec: Temporary Adhesive:
Peel Adhesion = .8lbs/sq inch Loop Tack = 1.2 lbs/sq inch May require adjustment for easier removal Adhesive sets up in 24 hours
3. DISREGARD - text page bottom page 12 – (CMOP Murfreesboro is no longer part of this BPA) The new Murfreesboro CMOP Facility is in the process of coming on line.
The Production equipment is not yet at full capacity.
CMOP fully expects the output capacity to increase as the production equipment and software are fined tuned to meet requirements.
An accurate forecast of increase production is not realistic at this time.
The Murfreesboro CMOP logistics team will work with the BPA Vendor on any expected increases in Label orders.
4. CLARIFICATION Page#7
• Should Fault be found to be defective PC Label, the Vendor will provide replacement PC Labels with a one day turn around, at vendor expense. In addition, the vendor is responsible for the collection and disposal of defective PC labels from CMOP.
"One day turnaround" to replace defective labels is not possible for any vendor to do correctly.
The Contracting Officer points to the requirement below as a solution to one-day turn around.
• To meet the above requirement, the Vendor will consider such approaches as developing and maintaining a disaster recovery plan or maintaining an emergency inventory.
Clarification: VA CMOPs understands that on high volume re-supply items a defective shipment may be delivered from the MFG. However, the critical part Labels play in the overall CMOP Mission cannot be emphasized enough.
Hence the requirement that the Vendor having an emergency inventory on hand for some such events. The make-up of the emergency inventory is up to the VENDOR.
· Whether it is a full month’s supply of backup Labels
· A two-week supply of backup labels that will last until the MFG can send replacement labels.
· A two-week supply of backup labels rotated out every quarter replaced with fresh backup.
Bottom line CMOP cannot run out of working labels due to a defective Label Lot.
The Vendor is responsible for replacing labels within one day turn around.
5. DISREGARD - text page bottom page 9 –

| 1st Quarter: 01 October – 31 December |
| 2nd Quarter: 01 January – 31 March |
3rd Quarter: 01 April – 30 June 4th Quarter: 01 July – 30 September
· Sales Reports will be a function of the Ordering Officers and COR. VISTA PO order will be cross referenced with IPPS Invoices submitted for payment.
