About this file
This document is Amendment 0001 to a Department of Veterans Affairs solicitation for laboratory testing services at the VA Network Contracting Office 23 in Sioux Falls, South Dakota. The amendment provides answers to 22 technical questions submitted by offerors regarding the Statement of Work and pricing structure.
Key clarifications include: all sample collection activities will be performed exclusively by VA personnel; up to 16 courier trips per year are included in the base bid with additional trips invoiced as required; monthly and quarterly testing will be conducted on the same day with four additional courier trips available for retests; only failing test parameters will be retested unless broader concerns exist; contractors must report actual numerical results for all tests, particularly failing endotoxin tests; contractors are responsible for reporting test results only, not root cause analysis or corrective action recommendations; participation in corrective action meetings is not required; twenty retests per year are included in the contract price with quantities marked as "Estimated" due to potential retests exceeding this threshold; retesting will be paid at the cost of one test per line item number; VA badges are not required for courier pickup; evaluation factors are not weighted but contractors must be technically acceptable to perform lab testing; specific technical submissions required include task breakdown worksheets, laboratory certifications, and past performance documentation; no incumbent contractor information is available; historical usage data is not available; contractor parking, loading access, and courier access are provided at no cost in designated areas; all bottles for required samples must be included in pricing; samples must be transported by courier, not through VA shipping warehouses; and sample pickup is anticipated for approximately 9:00 a.m. on scheduled collection days. All offerors must sign and submit the SF30 amendment with their quote.

CONTINUATION PAGE
Question 1 Solicitation Reference: SOW §7 Scope of Work Please confirm whether all sample collection activities will be performed exclusively by VA personnel, or whether contractor personnel may be required to perform collection under certain circumstances, such as retests.
All samples will be collected by VA Personnel
Question 2 Solicitation Reference: SOW §7 Courier Requirements Please clarify whether sixteen (16) courier trips represent the total annual requirement or only the minimum guaranteed quantity.
Up to 16 courier trips would be included in fees in original bid. Trips in excess of 16 are to be invoiced as required.
Question 3 Solicitation Reference: SOW §7 Courier Requirements It is anticipated, that the 16 courier trips would be distributed as 12 for each month of testing and the remaining for retests? Is it expected that the quarterly testing will be completed on the same day as the monthly testing?
Yes, they should be sampled on the same day. The four remaining trips would be used as needed for re-testing.
Question 4 Solicitation Reference: SOW §7 Retesting Requirements If a single parameter for one source is out of compliance and a retest is required, will just the single parameter be retested or will the complete panel be retested?
Only the failing tests would be re-tested, unless there was a broader concern.
Question 5 For reporting of endotoxin, is a result of “less than 10 EU/mL” or “greater than 10 EU/mL” sufficient on the report?
We expect to see the numbers associated with failing tests.
Question 6 Solicitation Reference: SOW Out-of-Compliance Procedures Please clarify the level of root cause analysis expected from the contractor.
Insight you have will be appreciated, but it is not in your scope of work to analyze root causes in our system and will not be compensated.
Question 7 Solicitation Reference: SOW Out-of-Compliance Procedures
Please clarify whether the contractor is responsible only for reporting findings or also providing corrective-action recommendations.
The Contractor is responsible for reporting test results only.
Question 8 Solicitation Reference: SOW Out-of-Compliance Procedures Please clarify whether participation in corrective-action meetings is required.
You are not required to attend meetings, but will be informed if additional testing is to be completed.
Question 9 Solicitation Reference: CLIN Schedule Please confirm that as a firm-fixed price contract and the unit being per month, that the monthly unit price quoted will be the price invoiced to the government regardless if all testing was performed? If this invoice would theoretically be rejected by the government, would the government consider changing the CLIN structure where
Per ANSI/AAMI ST108 and VHA Directive 1116, ALL tests will be completed monthly and quarterly as stated in the SOW, therefore the unit price will be the invoice price.
Per the SOW: Twenty (20) retests per year shall be included within the contract.
The 4 extra courier trip(s) were added due to the possibility of retests and could happen in the same month.
Quantities will be changed to “Estimated” due to the possibility of retests exceeding 20.
If additional retests, beyond the Twenty (20) are required, it will require Contracting Officer approval and a modification to the contract.
Question 10 Solicitation Reference: Retesting Requirements / CLIN Structure Please clarify how retesting costs should be proposed if no separate retesting CLIN exists.
Retesting will be paid for at the cost of one test as per each Line Item Number.
Question 11 Solicitation Reference: Security Requirements Please clarify whether contractor personnel who will be picking up samples will require VA badging, PIV credentials, facility access training, or background investigations.
Badges are not required for courier pickup.
Question 12 Solicitation Reference: Evaluation Factors Please clarify the relative weighting or order of importance among evaluation factors.
There are no weighted Evaluation Factors, however the contractor must be technically acceptable i.e. be able to perform the lab testing.
Question 13 Solicitation Reference: Evaluation Factors Please clarify what specific technical information the Government expects offerors to submit.
Task Breakdown Worksheet, Laboratory Certifications, Past Performance
Question 14 Please identify whether there is an incumbent contractor currently performing these services and if so, the previous contract number and/or price.
Information Not Available
Question 15 Solicitation Reference: Historical Usage Data Please provide historical annual testing and retesting quantities for the most recent contract year and for which measurements.
Information not available
Question 16 Solicitation Reference: Site Access Requirements Please clarify whether contractor parking, loading access, and courier access will be provided at no cost.
Contractor parking, loading access and courier access is available at no cost in designated areas.
Question 17 Solicitation Reference: B.2 Price/Cost Schedule When monthly and quarterly testing are aligned, will the monthly and quarterly samples be collected at the same time?
Yes.
Question 18 Will all monthly samples be collected on the same day to ensure only one courier visit is required each month?
Yes.
Question 19 Solicitation Reference: B.2 Price/Cost Schedule If a SPS source is not working at the time of routine collection, would the VA collect at a later time and require a second courier pick up that can be charged to the CLIN 0021?
Testing ports are available for sampling at each source whether equipment is in operation or not.
Question 20
Solicitation Reference: B.2 Price/Cost Schedule Several bottles are required for each source (1 for HPC, 1 for endotoxin, etc) such that more than 300 bottles will be required. Would the government consider removing CLIN 0022 and any costs for bottles will be included in the total price for CLINs 0001 to 0020?
Bottles for all required samples are to be included in the price.
Question 21 Would the government accept shipping samples to an accredited lab from the VA shipping warehouse as opposed to a courier?
No.
Question 22 What would be the predicted window in the day for sample pick up?
Pickup as coordinated – likely to be scheduled for approximately 9:00 a.m. on day of pickup.
