The amendment provides the evaluation factors that will be used to award a contract for Audiology Health Technician Services in the Minneapolis, MN area.
The key evaluation factors, in descending order of importance, are: 1) Technical Excellence, 2) Past Performance, and 3) Price/Cost. Technical Excellence includes sub-factors for personnel qualifications, clinical experience, and management/administrative capabilities. Past performance evaluation will consider relevant federal and commercial contracts from the past 3 years. The price proposal will be evaluated for fairness and reasonableness. EVALUATION FACTORS
a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
EVALUATION FACTORS:
The evaluation factors are listed in descending order of importance:
(1) Technical Excellence
(2) Past Performance
(3) Price/Cost
The sub-factors under each factor are of equal importance within the evaluation factor. The "Best Value" approach shall be used in the evaluation of proposals. The relative importance of technical excellence and past performance, when combined, are significantly more important than price.
NOTE: Offeror MUST submit a technical proposal as a separate document and MUST address each of the evaluation factors and sub-factors listed below in order to be considered for award:
FACTOR 1 - TECHNICAL EXCELLENCE
Offers will be evaluated on the sub-factors listed below. Offerors must submit the following requested documentation and information in the following format:
Technical Sub Factor A Personnel Qualifications: The offeror must provide the following documents / information demonstrating the qualifications of personnel to meet the requirements to include:
• A listing of all professional staff who will be assigned to perform the required services.
• Curriculum Vitae (CVs) of professional staff
• Copies of degrees; licenses; board certifications; accreditations
• A copy of current and active Basic Life Support (BLS), professional accomplishments and awards
• Credentialling will be required on the National Board for Certification in Hearing Instrument
Sciences (NBC-HIS) Certificate
Technical Sub Factor B Clinical Experience:
• The offeror must provide information regarding Audiology Technician ability to meet clinical requirements by providing information on experience to include:
1. Number of years of experience
2. Information regarding any special training
Technical Sub Factor C Contractor’s Management/Administrative Capabilities and Responsibility: The Technical Panel will evaluate the contractor’s management and administrative capabilities and responsibility:
The offer must provide information that demonstrates their company’s ability to provide required services and to manage and administer an awarded contract. This includes information regarding:
• Provide information that depicts evidence regarding qualifications and experience in management positions within the healthcare community. Include information on company’s financial stability. Does management possess the necessary organization and efficiency of operation to provide the personnel and to perform the services in support of an awarded contract?
• Provide information that corroborates your company has adequate personnel to provide required clinical services and the capability to maintain continuity of patient care in a timely manner. Include recruitment, staffing, back-up and retention plans.
FACTOR 2 - PAST PERFORMANCE
An offeror’s past performance is important in determining its ability to successfully perform required services. The government is especially interested in recent and relevant past performance that relates very closely to the requirement set forth in the performance-based work statement.
1. Provide a list of no more than five (5) of the most relevant contracts performed for Federal agencies and commercial customers within the past three (3) years, including name, address, telephone number, and email address of contract key personnel.
2. Provide documentation of any past/pending malpractice/tort claims.
3. Provide a list of three (3) professional references.
FACTOR 3 – PRICE/COST
The offer should contain the offeror’s best terms from a price standpoint. In determining the competitive range, price will be considered. Unreasonably low or incomplete proposed prices may be grounds for eliminating a proposal from competition either on the basis that either the offeror does not understand the requirement, or the offeror has made an unreasonable or incomplete price proposal.
For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but for no more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefor assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
Although the price proposal is lower than the others in importance, it will contribute to the source selection decision. The price proposal will be evaluated in terms of fairness and reasonableness. After an evaluation of the Technical Excellence, Past Performance criteria have been completed, price will be compared against these evaluations to determine the combination that is most likely to provide the best value to the Government, utilizing the tradeoff process.
