EVALUATION FACTORS:

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offer:

(1) Technical Excellence

(a) Personnel Qualifications

(b) Experience/Skills

(c) Collaborative Skills

(d) Contractor’s Management/Administrative Capabilities and Responsibility

Relative Importance: Technical Excellence and Past Performance will be afforded primary emphasis with Technical Excellence being more important than Past Performance. Sub-factors within the technical evaluation are equal importance. Evaluation factors other than cost/price, when combined, are significantly more important than cost/price.

FACTOR 1 - TECHNICAL EXCELLENCE

Offers will be evaluated on the sub-factors listed below. Offerors must submit the following requested documentation and information in the following format:

Technical Sub Factor A Personnel Qualifications: The Technical Panel will evaluate each candidate’s personal qualifications.

The offeror must provide the following documents /information demonstrating the qualifications of personnel to meet the requirements to include:

• A listing of all DRT staff who will be assigned to perform the required services.

• Copies of resumes and ARRT certification.

Technical Sub Factor B Experience/Skills: The Technical Panel will evaluate each candidate’s experience based on:

• Provide list of Imaging equipment that you can operate.

• Provide list and volumes of imaging exams you have performed.

• Provide years of experience 1 Year required.

Technical Sub Factor C Collaborative Skills:

• Provide three professional written letters of reference.

Technical Sub Factor D Contractor’s Management/Administrative Capabilities and Responsibility: The Technical Panel will evaluate the contractor’s management and administrative capabilities and responsibility:

The offer must provide information that demonstrates their company’s ability to provide required services and to manage and administer an awarded contract. This includes information regarding:

Provide information that corroborates your company has adequate personnel to provide required Imaging services and the capability to maintain continuity in staffing in a timely manner. Include recruitment, staffing, back-up, and retention plans.

FACTOR 2 – PAST PERFORMANCE

An offeror’s past performance is important in determining its ability to successfully perform required services. The government is especially interested in recent and relevant past performance that relates very closely to the requirement set forth in the performance-based work statement.

1. Identify the type and scope of practice for the past three (3) years addressing professional background, experience, and level of professional competence.

2. Provide a list of no more than five (5) of the most relevant contracts performed for

Federal agencies and commercial customers within the past three (3) years, including name, address, telephone number, and email address of contract key personnel.

3. Provide documentation of any past/pending malpractice/tort claims.

4. Provide a list of four (4) professional references.

FACTOR 3: PRICE

The offer should contain the offeror’s best terms from a price standpoint. In determining the competitive range, price will be considered. Unreasonably low or incomplete proposed prices may be grounds for eliminating a proposal from competition either on the basis that either the offeror does not understand the requirement or the offeror has made an unreasonable or incomplete price proposal.

For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but for no more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefor assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.

Although the price proposal is lower than the others in importance, it will contribute to the source selection decision. The price proposal will be evaluated in terms of fairness and reasonableness. After an evaluation of the Technical Excellence, Past Performance criteria have been completed, price will be compared against these evaluations to determine the combination that is most likely to provide the best value to the Government, utilizing the tradeoff process.
