About this file

This is a Past Performance Questionnaire form used to evaluate contractor performance on federal contracts. The form collects comprehensive information about a contractor's execution of a specific contract, including the contractor name, role, contract number, procuring agency, period of performance, dollar amounts (initial award, annual, and total with modifications), and contract type (Firm Fixed Price, Time and Materials/Labor Hour, Cost Reimbursement, or other).

The evaluation section requires assessors to rate contractor performance across seven key factors using defined adjectival ratings: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. Each factor must include mandatory comments justifying the assigned rating. The form includes a final question asking whether the evaluator would select the contractor again, with options for Definitely, Probably, or Definitely Not. The accompanying rating guidelines provide detailed definitions for five performance levels—Unsatisfactory, Marginal, Satisfactory, Very Good, and Exceptional—with specific criteria for each factor. This questionnaire serves as a standardized tool for documenting past performance history that informs future federal procurement decisions.
