30. Please confirm the maximum allowable length of stay under this contract. Is the initial limit 90 days, with extensions up to 6 months? Should contractors rely solely on Attachment D for extension parameters (SOW Reference 39.7; Attachment D)? The program is a 90 day contract to assist Veteran residents with transitioning into permanent housing. If for some reason the Veteran is actively searching for housing and something occurs out of the Veterans hands that delays their transition, they are eligible for up to 2 extensions beyond the 90 day time frame. Each can be from 1-3 month which is determined by the liaison; if both are granted for the max of 3 months, that is an additional 6 months in the program. Veterans that are not actively seeking housing are not eligible for extensions.
31. Clarify the workflow for same-day admissions versus next-business-day VA eligibility verification for consult, community, and direct referrals. Should contractors admit immediately before 7pm and obtain eligibility confirmation the next business day (SOW Reference 2.1.1; 2.1.1.1; 2.1.1.3; 2.2.2.1)? There are 2 ways for a Veteran to be admitted into the contract: 1- VA consults and 2- Direct referrals. For the VA consults, eligibility is already done if the veteran when they are referred for placement admission. For the Direct referrals, all contracts should enroll into SQUARES which they can determine eligibility. Staff should complete a SQAURES for each referred person; during VA business hours or by the next business contract staff must confirm eligibility with their assigned liaison. For both, the veteran is encouraged to present same day, however are given 24 hours in which the bed is held. If they do not present within that time frame, the bed is returned to beds available for placement.
32. Admission cutoff times differ (7pm vs. 9pm). Please confirm the official daily admission window. (SOW Reference 2.1.1.3; 2.4.1). All referrals should come in by 7pm, admission to 9pm.

(SOW Reference SOW 2.2; 2.2.2.1) This section, 2.2; 2.2.2.1 is specific to the Veterans being admitted. Veterans must be healthcare eligible but also be willing to engage in CM services, look for and secure housing and be ADL independent for placement consideration.
34. Is there a VA policy governing Safe Room monitoring standards, documentation, proximity requirements, or emergency thresholds (SOW Reference SOW 13.1.6)? in Section 13 of the contract it covers all above inquiries; this is considered a best practice model.
35. Are licensed nursing staff required for medication oversight, or may non-clinical staff monitor self-administered medications? Is VA responsible for any portion of medication reconciliation (SOW Reference 16.12)? No, an RN is not required. Contract staff do not dispense medication. Preparing 257 contract site, no additional space will be provided. Contractor is responsible to furnish the environment for it to be welcoming and the beautification of the space.
47. Will VA ensure all Life Safety Code requirements (fire extinguishers, alarms, signage) are met prior to contractor arrival, or must contractor procure missing items (SOW Reference SOW 8)? Currently the VA is addressing the bldg. to ensure it is meeting all code requirements, however all new contracts require an initial inspection before Veterans can be admitted; this inspection will consist of 5 disciplines who will evaluate the space after the contractor outfits the it with required staff, their policies and procedures, mock files/folder, livable accommodations for veterans to move in, etc.
48. Please confirm whether VA provides an after-hours point of contact for consult clarification, or whether contractors make all after-hours determinations until next business day (SOW 2.3). The contractor will have a CLINICAL SUPERVISOR and a PROGRAM MANAGER responsible for after-hours program needs. Liaison are consulted next business day.
