About this file

This document is Amendment 0002 to a federal solicitation for STAT Reference Lab Send Out Testing services for Phoenix Veterans Affairs Health Care System (VAHCS).

Solicitation Details: The solicitation number is 36C26226Q0675, issued by the Department of Veterans Affairs, Network Contracting Office 22, Long Beach, California. The requisition number is VA-26-0040889, with an effective date of May 13, 2026. The offer receipt deadline remains unchanged at 7:00 AM PST on May 25, 2026. This amendment addresses questions accidentally omitted from Amendment 0001 and clarifies solicitation requirements.

Key Service Requirements: The contractor must provide STAT (Stat) testing services for three tests marked as STAT in the requirements: ethylene glycol, volatiles, and methanol. Phoenix VAHCS reserves the right to request other tests as STAT in certain circumstances but not on a regular basis. Offerors must submit pricing and a Schedule of Services for all testing listed in Section B.2 Statement of Work, paragraph VIII List of Required Tests. The contractor must maintain continuous certification from the Centers for Medicare & Medicaid Services or accreditation by an approved regulatory agency (such as the College of American Pathologists) throughout the contract performance period. Performance standards include: specimen safety and integrity at 100% compliance with no lost or misplaced specimens; return of anatomic pathology materials within 7 days at 95% compliance; result reporting meeting turnaround times 90% of the time; 24/7 customer service accessibility with 90% voicemail follow-up compliance; timely monthly cumulative workload summary reports at 90% compliance; and proper certifications maintained at 100% compliance. The pricing proposal may use the provided Excel spreadsheet (optional) or an offeror's own format, provided all requirements and pertinent information are covered. Offerors may modify the attachment with additional columns for equal-to products or supplementary information.

STAT Reference Lab for Send Out Testing for Phoenix VAHCS.

The purpose of this Modification/Amendment is for the following:

1) To provide additional clarification to potential offerors on questions concerning this solicitation accidentally not addressed in Amendment 0001. See following pages for questions and answers.

2) Due date and time for receipt of offers remains unchanged. Due by: 7:00AM PST on 25 May 2026.

Question: Can you please confirm which tests would be required to run STAT? It looks like only 3 on the attached list are indicated as STAT? (ethylene glycol, volatiles, and methanol)

Answer: Yes, those 3 tests marked as STAT in the spreadsheet attachment (and also listed in section B.2. STATEMENT OF WORK, part VIII List Of Required Tests, page 12 of the solicitation) will always be STAT. The Phoenix VAHCS reserves the right to request others STAT in certain circumstances but not on a regular basis.

Question: on page 45. The document references “2.1.8 Offerors shall complete and submit the Schedule of Services in section B.3,” but I was unable to locate a section B.3 in the materials provided. I did notice there’s a ‘Schedule of test performance’ listed under B III A 13 on page 5, which appears to align more closely with the overall statement of work. Could you clarify if this is the section we should be referencing, or if there’s a separate document or section we might have missed? Thank you for your assistance.

Answer: Section B3 does not exist, clerical error. The correct language should have read “Offerors shall submit their Schedule of Services and pricing for testing listed in section B.2 Statement Of Work, paragraph VIII List of Required Tests”. Essentially, to provide one’s pricing/quote.

Question: We would like to know if this is a brand new contract OR if there is (was) an incumbent performing these services. If not brand new, could you please provide the current / previous contract number?

Answer: This question is not reflective of the solicitation. We are only answering questions that fall within the scope of the solicitation post.

Question: The General Instructions for Proposal Submission indicate submissions should be in Adobe or MS Word formats. Please confirm whether the pricing proposal may be submitted using the provided Excel spreadsheet (S02 – RFQ Attachment A RefLab.xlsx).

Answer: The purpose of the attached spreadsheet with the solicitation is for offerors to use it for their pricing/offers/quotes to the government but is optional and not mandatory. If an offeror wishes, they may use their own quote format and letterhead with pricing information instead of the spreadsheet.

Question: Can you please confirm whether it is acceptable to add additional columns to the far right of the Attachment A document to include the following information?

Answer: If an offeror wishes to modify Attachment A spreadsheet to provide equal-to products and or additional information, they may. As long as their proposal covers all requirements and shows all pertinent information. Another option is for an offeror to use their own quote format and letterhead with pricing information instead of the spreadsheet, once again, ensuring it covers all required items.

Question: Section VII – Quality Assurance Monitoring, Item E references a Performance Requirements Summary Matrix; however, this matrix does not appear in the posted documentation. Should offerors assume the same QASP matrix used for VA Phoenix Sources Sought 36C26226Q0388?

Answer: Yes, the matrix did not move over to the solicitation. See below.
