About this file
This is an Amendment to Solicitation/Modification of Contract (Standard Form 30) issued by the Department of Veterans Affairs Network Contracting Office 22 on May 4, 2026.
Amendment 0001 to solicitation 36C26226Q0384 modifies the solicitation to update the Statement of Work (Attachment B) to reflect Arizona as the location instead of California, attach wage determination documents for each location's county, provide an Attachment C Business Associate Agreement (BAA) Sample Template, and answer ten vendor questions. The amendment clarifies that all insurance requirements—Workers' Compensation, Employer's Liability, and General Liability—shall be provided in accordance with Arizona law. The amendment confirms that estimated mileage figures in the pricing attachment represent either one-way or round-trip distances depending on the specific CLIN (Contract Line Item Number), with CLIN 1 as an example of round-trip and CLIN 7 as one-way. For STAT (emergency) courier services, which must be available 24 hours per day, 7 days per week, 365 days per year, offerors are instructed to incorporate all costs of maintaining on-call readiness and availability into their per-mile pricing for the STAT CLIN, with no separate standby or availability fee line item permitted. The amendment provides detailed responses regarding bin quantities and dimensions for various departments (pharmacy, mailroom/logistics, and surgical processing systems) across three Arizona locations: Prescott, Phoenix, and Tucson.

7. Business Associate Agreement (BAA): The solicitation requires offerors to "complete and sign the Business Associate Agreement" and submit it with the quote, but a BAA template does not appear to be included in the posted solicitation package. Would the
Contracting Office please provide the required BAA template, or confirm the form the Government expects offerors to use?
-BAA Sample Template attached.
8. Insurance Requirements – State of Jurisdiction: The Statement of Work states that
Workers' Compensation, Employer's Liability, and General Liability Insurance shall be provided "in accordance with the laws of the State of California," while all listed places of performance are located in Arizona. Could the Government clarify whether insurance shall be carried in accordance with California or Arizona requirements, or both?
-An updated SOW is attached to reflect the change from California to Arizona.
9. STAT Run Pricing (CLIN 92): Attachment A reflects a single STAT CLIN with an estimated annual quantity of 1 and estimated mileage of 1, while the SOW requires
STAT courier services to be available 24 hours per day, 7 days per week, 365 days per year. Could the Government clarify how offerors are expected to reflect the cost of maintaining 24/7/365 on-call STAT capacity within a single per-mile CLIN, and whether any additional line item (e.g., standby or availability fee) is permitted?
- Offerors are encouraged to incorporate the costs of maintaining on-call readiness, including any standby or availability expenses, into their per-mile pricing for the STAT
CLIN. At this time, no separate line item—such as a standby or availability fee—will be established for this requirement. All anticipated costs related to availability and service readiness should be reflected within the per-mile price proposed for the STAT CLIN.
10. Estimated Mileage Basis: For the mileage figures in Column F of Attachment A (tabs 1–
5), can the Government confirm whether the listed "Estimated Mileage" represents a one-way or round-trip distance for each run, for the purpose of calculating
Extended Price using the Unit Price Per Mile?
-It represents either one-way or round-trip depending on the CLIN(RUN). Example CLIN
1 shows a round trip while CLIN 7 is one-way.
