About this file
This is a Justification for Single Source Awards under Revolutionary Federal Acquisition Regulation Overhaul (RFO) 12.102(a) for a contract modification over the micro-purchase threshold but not exceeding the simplified acquisition threshold.
The Department of Veterans Affairs (VA), Veterans Integrated Services Network (VISN) 22, Phoenix VA Health Care System (PVAHCS) is modifying existing Indefinite Delivery Contract (IDC) No. 36C26226D0057 to add Nuclear Medicine teleradiology interpretation services. The Purchase Requisition is numbered 644-26-4-047-0779 in the amount of $35,325.00, with an effective date of December 30, 2025. The modification is carried out by the Network Contracting Office (NCO) 22 under Acquisition Plan Action ID 36C262-26-AP-7506. The immediate requirement is to provide temporary Nuclear Medicine interpretation coverage for PVAHCS starting July 15, 2026 through August 19, 2026, due to the unavailability of a VA-employed Nuclear Medicine Radiologist. The modification adds Nuclear Medicine interpretation services to the existing contract as an available service for future use by PVAHCS and Northern Arizona VA Health Care System (NAVAHCS), with individual task orders determining actual service ordering.
The justification cites unique characteristics limiting availability to one source: the incumbent contractor has already established credentialing, secure connectivity, reporting workflows, and integration with VA imaging systems. Awarding services to another contractor would require duplicative credentialing, onboarding, systems integration, and workflow implementation, creating operational risk during the critical coverage period. The government previously conducted market research in support of the original IDC award and identified capable commercial teleradiology vendors; however, no additional market research was conducted for this modification action as it modifies the existing contract. The government plans to pursue a VISN-wide competitive acquisition strategy for long-term teleradiology services in the future, with acquisition planning and requirements development underway to provide vendors sufficient lead time for credentialing and technical onboarding.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The requested Nuclear Medicine teleradiology interpretation services are being added to an existing contract under which the contractor already provides teleradiology interpretation services for PVAHCS and NAVAHCS. The incumbent contractor has established credentialing, secure connectivity, reporting workflows, and integration with VA imaging systems necessary to provide uninterrupted teleradiology services.
Awarding these additional Nuclear Medicine interpretation services to another source would require separate credentialing, onboarding, systems integration, workflow implementation, and administrative oversight. This would duplicate existing efforts, increase transition time, and introduce unnecessary operational risk during a period when PVAHCS requires uninterrupted coverage due to the anticipated absence of its sole Nuclear Medicine Radiologist. Modifying the existing IDC to include these services is the most efficient and effective means of meeting the Government’s requirements while maintaining continuity of patient care.
4. Description of market research conducted and results or statement why it was not conducted: Market research previously conducted in support of the award of IDC No.
36C26226D0057 identified commercial teleradiology vendors/providers capable of performing the required services. However, the current requirement is limited to adding Nuclear Medicine teleradiology interpretation services to the existing contract for PVAHCS alone.
The Government considered the existing contract and the current operational requirement in determining the appropriate acquisition approach. This action modifies the existing IDC to add Nuclear Medicine teleradiology interpretation services required by PVAHCS. Because the Government is modifying the existing contract to meet this requirement, no additional market research was conducted for this action.
Awarding this limited requirement to a different contractor would require duplicative efforts, which could delay implementation and increase administrative burden. Therefore, modifying the existing contract is the most efficient and effective means of meeting the Government’s requirements.
To remove these barriers and promote competition for future requirements, the Government is pursuing a VISN-wide competitive acquisition strategy for long-term teleradiology services.
Acquisition planning has been initiated, and requirements development is underway. The planned competitive solicitation will provide sufficient lead time for interested vendors to complete credentialing, privileging, and technical onboarding requirements.
5. Contracting Officer's Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
