1. What is the full listing of VA Directives for the compliance review? VHA Dir 1060,

2. Will previous TJC review/accreditation surveys be available for review?None. this is a new requirement since previous surveys.

3. Can you please provide a copy of the water management plan (WMP)? It was our understanding that there currently is no WMP for the facility but instead they have different

WMPs for various departments. i.e. SPS, critical water etc. there is not a WMP.

The assessment will be used to create the water management plan.

4. If a complete WMP is not available (ie non-existent), the SOW will need to be revised to reflect preparation of a WMP. In that context, can you please advise on the status of the following WMP elements: Based on the water systems at VA San Diego Healthcare System which includes the following:

1) Domestic Hot

2) Domestic cold water

3) Closed loop chilled water to the air handler

4) Closed loop chilled water to the cooling towers

5) Closed loop heating hot water

6) Steam supply to sterilizers

7) Dialysis water

8) Critical water

a) Research Department

b) Lab Department

c) Sterile Processing Department

9) Therapy Pool

10) Irrigation

11) Dental water

a. Does a water management team exist? How frequently do the meet and what is their training? If so, how long ago? Yes. Quarterly

b. Do water system maps exist for all buildings/floors? Do they contain water flow diagrams? If so, how long ago? Yes. Initial construction of each system

c. Have risk assessments been performed for all risk factors and processes? If so, how long ago? Part of the assessment to review that

d. Have control measures been developed for all processes? If so, how long ago? Part of the assessment

e. Is data being collected and reviewed consistently? If so, when was the last data set?

Is data collection (water sampling) a part of this scope? Assessment should determine that

5. The SOW is limited to an audit of the WMP. Is it correct that there is no analytical sampling or mechanical systems analysis in the SOW (e.g. identification of dead legs)? Not deadleg recommendation, but assessment of inspection, testing, maintenance of mechanical systems

6. Requirements for key personnel are not provided. Can the VA clarify if a CIH or PEs are required or desirable given complexities in WMP management? CIH andPEs required.

Please, recommend expertise certifications based on water systems provided.

7. What is the full listing of buildings included in the WMP? 1, 2, 11, 14, 23, 25, 27, and offsite clinics(ASPIRE, Kearny Mesa Clinic(dental water), Oceanside Clinic(Dental Water)

8. What specialized areas of the facility are included that are not associated with potable water consumption (e.g., critical water, dental water, irrigation systems)?critical water, dental water, hemodialysis water system, therapy pool, irrigation system, close loop chilled water to air handlers, closed loop chilled water to water cooling towers, steam supply,

9. What VHA standards are included in the WMP (e.g., ES-2019-001)? VHA Dir 1060, VHA Dir 1116, Physical Therapy for Therapy Pool, applicable standards for Lab and Research use water, AAMI guidelines for dialysis.

10. Given the lack of details in the SOW, please consider extending the due date by 2 weeks.

11. Was this service performed under an earlier contract? If so, can the contract number be provided? no
