1. The purpose for this amendment is to respond to question number 22. All amendments shall be acknowledged and returned with offer.

Questions and Responds Courier Service

1. As a top view, it looks to be one pick per day (M-F) at each of the three locations. Is that correct? Or is there a return delivery to each of the three locations after dropping off at 3350 La Jolla Drive?

ANSWERS: That is correct. One pickup at the end of each CBOC’s service day excluding weekends and holidays and no return deliveries.

2. I see pickup times of 4:30 and 5:00pm for Oceanside, Chula Vista, and Kearny Mesa. I assume these are then to go directly to the VA in San Diego at 3350 La Jolla Village Drive?

ANSWERS: Yes, that is correct

3. Would the attached 2500 Promaster Van be of ample size for each pick up? Usable interior space is 126”L x 60”W x 72”H.

ANSWERS: Yes, it is. The van must be temperature controlled and have ample space for the red biohazard transport bins that will be picked up from each CBOC location.

4. Can you clarify the Option distinctions (Option 1, Option 2, and Option 3) please?

ANSWERS: Price increase is the only difference for the option years. The option years are for continuous service so there is no lag is transportation.

5. I believe it to be, plainly, (250) roundtrip transports for the year term, for each location Or (750) roundtrips total. Is that correct?

ANSWERS: That is not correct. There is only one pick up per day-- No round trips. Each pick up is schedule to exclude the weekend and holidays for a total of 250 pickups each CBOC one way. The pricing of each is different based on the distance to 3350 La Jolla Village Dr.

6. What materials will need to be picked up so I can assess what vehicles will need to be used. I saw "RMD" listed on the contract but don't know what that is.

ANSWERS: RMD is Reusable Medical Devices. These devices are used for outpatient procedures that the CBOCs are able to perform at their location. Because these items have bioburden on them (blood, bone, body fluids, etc), and are they must be picked up from the CBOCs and brought back to VA San Diego Sterile Processing Service to be decontaminated and sterilized for the next day usage. It is imperative that we get the soiled instruments back after use before the CBOC closes because the bioburden can harbor bacteria that could be life threatening to the Veteran and damage the reusable devices. They are returned in, locked rigid red containers identified with biohazard markings.

7. Could you provide me with a schedule, locations to and from etc?

ANSWERS: Please see schedule below provided in the SOW. I have also attached it to the email.

ANSWER: The previous contract number is 36C26218D0130

9. What are the number of miles between each pick-up and delivery locations?

ANSWERS: There is no specific route from the CBOC to San Diego VA. We have no mileage information.

10. What is the cost of the final full option year of the previous contract?

ANSWER: total: $54,372.48 (12 month) What’s the name of the previous awardee’s? (Who is incumbent contractor) ANSWER: Innovation Federal Operations Group Incorporated

11. What are the maximum weights for the supply pickups?

ANSWER: No single item over 25 pounds. Has never exceeded 300 lbs.

12. How many approximate boxes per location?

ANSWER: 3-6

13. Is this a new contract?

ANSWER: Yes

14. How many drivers are currently being used?

ANSWER: 2 or 3, you would have to verify, with the provider.

15. What is the weekly, monthly, or annual estimated trip volume for trips in the total, and/or as-needed trips per contract location?

ANSWER: 250 trips to each, of the 3 CBOCS (Oceanside, Chula Vista, and Kearney Mesa)

16. If pick up time is 5pm what is the latest time they can pick up from Oceanside CBOC and deliver at VASDHS? Can you please specify what time for pickup and drop-off?

ANSWER: Each CBOC “Pickup” time is listed in answer #7…there is no drop off specified time…usually happens before 2000hrs./daily.

17. If pick up time is 5pm what is the latest time they can pick up from Kearny Mesa CBOC and deliver at VASDHS? Can you please specify what time for pickup and drop-off?

ANSWER: Each CBOC “Pickup” time is listed in answer #7…there is no drop off specified time…usually happens before 2000hrs./daily.

18. If pick up time is 4:30pm what is the latest time they can pick up from Chula Vista CBOC and deliver at VASDHS? Can you please specify what time for pickup and drop-off?

ANSWER: Each CBOC “Pickup” time is listed in answer #7…there is no drop off specified time…usually happens before 2000hrs./daily.

19. Regarding VA Form 10091 and our company’s registration with the VA Financial Service Center: Is it acceptable to include a copy of VA Form 10091 with the proposal as evidence, considering we have an existing registration and active status with the VA Financial Service Center?

ANSWER: Yes, it is acceptable to receive a copy of your forms with your offer.

20. Do you accept CPARs evaluations as additional Past Performance information?

ANSWER: Yes, it is acceptable to receive a copy of your CPARS evaluations.

21. Regarding specific employees information: It is common practice for small businesses that the drivers, cars, and other equipment used for a contract are not sourced until after an award has been made, for cost effectiveness and to reduce loss. However, we will be forwarding our standard Training Certificates-Procedures for your reference, is this information acceptable?

ANSWER: Yes

22. Are the times that are listed firm? Meaning is the driver required to be there at 5pm (for example) or is this more of a “don’t arrive prior to…5pm”? We are attempting to determine if one dedicated van can accomplish this or will two be needed to arrive at the 2 locations that have the same pick-up times.

ANSWER: Good question….it is a “firm – do not arrive before….5pm.” because clinics are may still be operating and the dirty RMD would not be available before 5pm.

Also – it is critical that no RMD is left overnight, at the CBOCs.
