Questions & Answers

Questions have been combined; meaning if the same question or like question was submitted by multiple venders, they are all listed prior to the answer, which is RED font.

· Is there a set pricing schedule or template you wish to be followed in the proposal? If not, do you want the cost itemized in an hourly cost fashion differentiating between regular/afterhours/weekend hourly rates?

Services provided Monday – Friday, 8:00 am to 5:00 pm Please provide pricing that includes:

Same-Day Delivery Service:

4-hour Delivery Service:

2-hour Delivery Service:

Include any pricing that your company itemizes, such as:

Wait time rates Mileage rates After hour rates If you are available on weekends, if so, what are the rates

· Is this solicitation a contract renewal or a newly desired service? If a renewal, is it possible to share the previous awarded contract notice ID #?

What was the cost of the final full option year of the previous contract?

How many drivers/vehicles are currently used to service the current contract?

What is the vehicle size currently servicing this location?

This is a new requirement.

· I did not see a limitation on the services needed as far as working after the normal workday (after hours) and weekends. Are you only looking for normal business hours for services? Or being able to respond 24/7 (except holidays)?

What is the weekly, monthly, or annual estimated trip volume for scheduled trips in total and what is the weekly, monthly, or annual estimated trip volume for as-needed trips?

This is for delivery to/from the main VAMC in Phoenix, Az to/from the clinics (CBOC’s) it is affiliated with. The VA may borrow out-of-stock medications/supplies from local community hospitals with Maricopa County. See the map below of CBOC locations:

Answer from facility SME: We anticipate trips to our Show Low and Globe Clinics at least twice a month as scheduled trips. We also anticipate local trips to our CBOC locations at least 3-4 times a week to restock refrigerated medications and vaccines as needed. So, I would say we estimate 6 scheduled and 12 -15 as needed trips a month if we go by our current need. We've never used a courier before, so these are pure estimates.

· Are the Courier services your requesting based at Carl T. Hayden VA Medical Center and other community hospitals nearby? If so, where is the location limitations drawn? Is this service limited to all Phoenix city limits, or county limits?

See previous question & answer.

· Regarding specific employees’ information: It is common practice for small businesses that the drivers, cars, and other equipment used for a contract are not sourced until after an award has been made, for cost effectiveness and to reduce loss.

This requirement is needed to immediately after award in order to prevent further medication delay or spoilage. Once offers are received I will all announce the accepted offer prior to final award, that will allow a period of time to set things in place.
