Statement of Work

Introduction

The purpose of this Statement of Work is to outline the tasks and responsibilities in relation to the VA Greater Los Angeles Healthcare System (GLA, CBOCs, ACC’s and Clinics) purchase of services: delivery of medical cylinders to the VA Greater Los Angeles Healthcare System (GLA, CBOCs, ACC’s and Clinics) in support of patient care.

Scope

The contractor shall provide medical cylinders to VA Greater Los Angeles Hospital, associated VA CBOCs, associated VA ACC’s, and other VA Clinics within 48 hours of receipt of order. Contractor must be able to deliver emergency orders within 6 hours of receipt of order. Delivery Schedule

Deliveries at the VA Greater Los Angeles Hospital will be to the medical cylinder cage on the loading dock between 0930 am PST and 1030 am PST. Deliveries outside of that window will not be accepted and no charges can be invoiced. All deliveries to VA CBOCs and Clinics will be smaller in size and less frequent so delivery time will be determined by the Point of Contact at the VA CBOC or VA Clinic. All Points of Contact at the VA Greater Los Angeles Hospital, VA CBOCs, and VA Clinics will be provided to the winning contractor.

Period of Performance

The Period of Performance is Monday through Friday, April 1st, 2019 to March 31st, 2020, excluding all weekends and federal holidays. Federal holidays are:

· Labor Day (First Monday in September).

· Columbus Day (Second Monday in October).

· Veterans Day (November 11).

· Christmas Day (December 25).

Emergency orders will be required on weekends and holidays, but the contractor is required to include a cost for emergency deliveries in their offer if they intend to charge for it.

Contractor Responsibilities

The contractor will provide the VA Greater Los Angeles Healthcare System with an order form that contains a list of the medical cylinders on contract, a space for the quantity required by the end user, a space for the delivery address and location, a space for the delivery Point of Contact with cell phone number, and a space to list the Purchase Order number. The end user must be permitted to fax or email the order form. The contractor will acknowledge within 1 hour of receipt of delivery order through phone call or email. The contractor is responsible to correct any issues immediately that are covered under the contract. The contractor is responsible to communicate in advance with the VA Greater Los Angeles Healthcare System staff any unusual lead and turnaround time periods. The contractor must upload all invoices with the associated Proof of Delivery for invoices related to products delivered. A monthly invoice for Rental Charges must be accompanied by a spreadsheet that contains a list of all cylinders currently in possession of the VA Greater Los Angeles Healthcare System to include the serial number, current location (hospital, clinic), contracted price, and return date if returned.

VA Greater Los Angeles Healthcare System Responsibilities

The VA Greater Los Angeles Healthcare System will email order form to the contractor 48 hours in advance of required delivery. The VA Greater Los Angeles Healthcare System shall provide planning and communication on all medical cylinders required. The VA Greater Los Angeles Healthcare System staff will certify all approved invoices for deliveries of medical cylinders.
