B.X STATEMENT OF WORK / DESCRIPTION
1. CONTRACT TITLE:
Repair of 6th Floor Walk-in Refrigerators at VA San Diego Healthcare System (VASDHS).
2. PLACE OF PERFORMANCES:
VA San Diego Healthcare System Research Department 3350 La Jolla Village Dr.
San Diego, CA 92161
3. PERIOD OF PERFORMANCE:
After award, 60 – 90 days for completion to allow for materials delivery.
4. CONTRACT TYPE: Firm Fixed Price (FFP)
5. BACKGROUND:
VASDHS has various walk-in refrigerators that are located throughout the Research Department on the 6th floor. These walk-in refrigerators are between 15-20 years old. The purpose of these walk-in refrigerators is to store research samples, materials, and specimens with temperature ranges from 36 to 40 degrees Fahrenheit. Currently, some are operational while others are non-operational. As such, VASDHS is requesting repairs to these refrigerators to have it 100% operational.
6. GENERAL REQUIREMENTS:
6.1. Contractor shall furnish all equipment, incidentals, labor, materials, parts, supervision, supplies, tools, and qualified personnel necessary to repair / replace six (6) walk-in refrigerators located throughout the 6th floor. Contractor shall remove / replace evaporator coils, water cooled condensers, and recover / dispose of refrigerant. Evaporator coils and condensers must match so they are can operate correctly and efficiently. For each walk-in refrigerator, the contractor will do the following:
6.1.1. All new materials shall be compatible with refrigerant 404A.
6.1.2. Contractor shall field verify all equipment compatibility.
6.1.3. Purchase new matching water cooled condenser w/head pressure control valve and water-cooled evaporator coil (115V, 1 phase) that meets current industry standard.
6.1.4. Disconnect power to each unit and perform proper tag out.
6.1.5. Recover and dispose of refrigerant in the systems.
6.1.6. Remove all piping to the condensers and coils.
6.1.7. Remove old condensers and evaporator coils.
6.1.8. Install new condensers and evaporator coils.
6.1.9. Re-connect all piping to condensers and coils.
6.1.10. Re-connect line power and control wiring.
6.1.11. Pull vacuum and charge the units with current industry refrigerant.
6.1.12. Perform leak test.
6.1.13. Power up each unit and ensure fully operational.
6.2. Special Note: The condenser units are in the interstitials, as such, contractor will need to hand carry the old and new units through this space. Furthermore, contractor doing this part of the project must have the Asbestos Training and Respirator fit test as outline in paragraph 7.
7. CONTRACTOR QUALIFICATIONS & OTHER RESPONSIBLITES:
Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government.
Furthermore, contractor and contractor’s employees performing on this contract shall have, but not limited to the following qualification, experience, licenses, and certifications:
7.1. Contractor and/or contractor’s employee(s) performing on this contract shall be asbestos certified because VASDHS has asbestos in the building. This asbestos requirement only applies to personnel who will do the physical installation of the equipment and peripheral parts. All other personnel that will not disturb or contact asbestos, will not be required to have an asbestos certification. Contractor shall provide the following certificates for assigned personnel prior to the performance of the contract:
7.1.1. “Current 16-hour Operation and Maintenance Asbestos Training” &
7.1.2. “Current Respirator fit test”.
The Government reserves the right to accept or reject Contractor’s employee for the rendering of services. Complaints concerning Contract Personnel’s performance or conduct will be dealt with by the Contractor and COR with the final decision being made by the Contracting Officer.
8. SAFETY REQUIREMENTS:
8.1. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract.
8.2. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property.
8.3. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers’ health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137
8.4. Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely.
9. CHANGES:
The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery.
In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof.
10. VA POLICY:
Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court.
11. PARKING POLICY:
It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor’s personnel under any circumstances.
12. IDENTIFICATION BADGES:
All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented.
13. SMOKING POLICY:
Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas.
14. NORMAL WORKING HOURS:
Contractor will shall be performing services between the hours of 7:30 AM through 4:00 PM Monday through Friday, with exceptions to National Holidays.
15. OVERTIME AND NATIONAL HOLIDAYS:
Any overtime or holiday pay that may be entitled to the Contractor’s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.
The Contractor is not required to provide service on the following U.S. Government holidays nor shall the Contractor be paid for these days:
