About this file
This document is an Amendment to a Request for Quotation (RFQ) for a Direct-to-Patient (DTP) Delivery Service of Medical/Surgical Supplies issued by the Department of Veterans Affairs Network Contracting Office. The solicitation (36C26125Q0406) is for a multi-year, single-award Indefinite Delivery/Indefinite Quantity (IDIQ) contract within the VA San Francisco Healthcare System, with an initial performance period from 10/01/2025 to 09/30/2026. The amendment extends the solicitation response date to 07/03/2025 at 15:00 EDT and clarifies several questions from potential offerors.
The solicitation will be evaluated across six factors: Direct-to-Patient Delivery Capabilities, Delivery Timelines, Price, Authorization (requiring OEM or authorized distributor status), Past Performance, and Service-Disabled Veteran-Owned Small Business (SDVOSB)/Veteran-Owned Small Business (VOSB) Status. Offerors are required to submit a comprehensive catalog with at least 20,000 medical-surgical items and demonstrate the ability to deliver large volumes of supplies directly to patient homes. The government will use a comparative evaluation process to select the "best" offeror, with potential preference given to SDVOSB/VOSB businesses, and reserves the right to award based on factors beyond just the lowest price.

· Key contractor personnel will need to be identified at time of award.
· Is the intent to set-aside the award in whole or in part to Cardinal Health at-Home or its subsidiaries?
· Cardinal Health is the brand name example for the CMOP/DTP services required. Offeror does not need to be affiliated with Cardinal Health.
· To be technically acceptable, must an offeror propose a catalog containing at least 20,000 medical-surgical items?
· The Government is looking for an extensive catalogue to formulate an IDIQ against to fulfill DTP needs.

E.14 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Factor 1 – Proposed DTP – The Government is evaluating for the capabilities of offerors to provide a comprehensive DTP service. Due to the potential quantity of line items, the Contracting Officer and Subject Matter Experts are requesting a comprehensive price list/catalogue of supplies to be included in the response. Offerors proposing “equal” items shall submit sufficient descriptive literature to permit a thorough evaluation of the item’s characteristics, including design, materials, components, and performance characteristics, to determine equivalency to the brand name item specified UPON VA REQUEST. The Government may find more preferable any offer that shows additional quality and durability features of items.
Factor 2 - Delivery – The Government is assessing conceptual understanding of the proposed DTP service delivery timelines. The Government is assessing the offeror's ability to deliver large volumes of supplies directly to patient homes within the VA San Francisco Healthcare System. "Ability" refers to how quickly orders are fulfilled and if there are any risks of unfulfillment. Please see the “Requirements” section of the SOW.
Factor 3 - Price – The Government is evaluating price for fair and reasonableness.
Factor 4 – Authorization – Quotes provided must be from the Original Equipment Manufacturer (OEM) or an authorized distributor. Authorized distributors must provide proof of authorization in order to be considered for award, failure to do so may lead to the Contracting Officer determining the quote incomplete.
Factor 5 – Past Performance – The Government will be reviewing the offeror’s prior experience in performing this type of requirement. Offerors may submit a list of past contracts for reference: the list should contain contract numbers, contract descriptions, dollar values, and Government POCs.
Factor 6 - SDVOSB/VOSB Status - This factor will be used to assess the Offeror’s status as a Service-Disabled Veteran-Owned Small Business (SDVOSB) or Veteran-Owned Small Business (VOSB) concern, or the Offeror’s proposed use of eligible SDVOSB/VOSB concerns as subcontractors. SBA.gov verified veteran socioeconomic status of offerors MAY provide more favorability during the evaluation process.
i. To receive credit as an SDVOSB/VOSB concern, Offerors must:
1. Provide a statement claiming credit as an SDVOSB/VOSB concern, as appropriate;
2. Provide a statement certifying that the Offeror meets the size standard to be classified as a “small business” for the NAICS code assigned to this procurement;
3. Provide the Offeror’s full legal name as that name appears in the Offeror’s registration in the SBA.gov database (available at https://www.sba.gov/) and SAM UEI; and
4. Be registered and listed as “Verified” as an SDVOSB/VOSB concern in the SBA.gov database (available at Small Business Administration (sba.gov)) on the date offers are due.
5. Note that SDVOSBs get full credit, VOSBs get partial credit and non-veteran businesses proposing SDVOSBs and VOSBs as subs will get some consideration.
ii. To receive partial credit for proposed use of eligible SDVOSB/VOSB subcontractor(s), the proposed subcontractor(s) must be registered and listed as “Verified” in the database (available at Small Business Administration (sba.gov) ) on the date offers are due and must meet the size standard to be classified as a “small business” for the NAICS code assigned to this procurement. To receive credit for proposed use of eligible SDVOSB/VOSB subcontractor(s), Offerors must submit the following information with their offer:
1. The Offeror must provide a statement claiming credit for use of an eligible SDVOSB/VOSB subcontractor for which Offeror seeks to claim such credit;
1. The Offeror must provide the following information regarding its proposed SDVOSB/VOSB subcontractor(s):
1. The subcontractor’s full legal name as that name appears in the subcontractor’s registration in the database and SAM UEI;
1. A statement certifying that the proposed subcontractor(s) meets the size standard to be classified as a “small business” for the NAICS code assigned to this procurement;
1. A narrative that describes the specific tasks each SDVOSB/VOSB subcontractor will perform in support of this requirement where credit for use of SDVOSB/VOSB subcontractors is claimed;
1. A statement of the estimated dollar value of the subcontract that will be awarded to each SDVOSB/VOSB subcontractor where credit for the use of SDVOSB/VOSB subcontractors is claimed;
1. For each SDVOSB/VOSB subcontract claimed, the Offeror must provide a letter of intent signed by an authorized representative of the SDVOSB/VOSB subcontractor and signed by the Offeror committing the SDVOSB/VOSB subcontractor to perform the work described in (b)(iii) above and committing the Offeror to subcontract with the SDVOSB/VOSB concern to perform that work if the Offeror is awarded a contract under this procurement. The letter must specify the specific tasks the subcontractor will perform, the physical location where the work will be performed, the period of performance for the subcontract, and the approximate dollar value of the subcontract.
This evaluation factor will be evaluated for Full Credit / Partial Credit / No Credit.
The information requested above is required for the Government to evaluate offers for the SDVOSB/VOSB Status evaluation factor. General statements claiming that an Offeror will use eligible SDVOSB/VOSB subcontractors to perform work under this procurement or that do not include the information required above are not acceptable. FAILURE TO SUBMIT THE INFORMATION REQUIRED ABOVE, INCLUDING THE SMALL BUSINESS CERTIFICATIONS AND THE LETTER OF INTENT SIGNED BY BOTH PARTIES, MAY BE VIEWED BY THE GOVERNMENT AS EVIDENCE THAT THE OFFEROR SHOULD RECEIVE NO CREDIT WHEN EVALUATING OFFERS FOR THIS FACTOR.
Quotes will be evaluated by performing a comparative evaluation in accordance with FAR 13 to determine which response represents the “best” as a whole. Quotes will be evaluated by performing a direct comparison of one offer with another in a uniform manner to determine which quote provides the government with its needs, as identified in the RFQ. The government reserves the right to consider a response that offers more than the minimum and select that response if it provides a benefit to the government.
(b) Options. For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows:
The evaluation will consider the possibility that the option can be exercised at any time, and can be exercised in increments of one to six months, but for no more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be [at the same rates as those in effect under the contract at the time the option is exercised.] The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise.
This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
(c) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation, but is not required to do so. Each response must meet the minimum requirements of the solicitation.
A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
