RFI #1 – Please confirm how Proposals shall be submitted as there appears to be conflicting direction:

Section 1.6 Note 2 on page 8 of 72 of Solicitation document specifies that Offerors shall not submit form SF 1442 as part of Phase 1 and that no cost /pricing information shall during phase 1 of the selection process, but in section 1.7 Selection Procedures it states, “the initial Phase-One shall conform to the solicitation requirements and shall contain the best offer”. Please confirm the 12-3-2021 date identified on SF 1442 is for the Phase 1 selection criteria package only.

VA Response: Cost/pricing information shall not be submitted with Phase-One proposals.

Regarding the statement “the initial Phase-One proposal shall conform to the solicitation requirements and shall contain the best offer,” during Phase-One evaluations, the Government does not intend to hold discussions with any offerors; therefore, please ensure that Phase-One submissions, specifically Factor 2, Technical/Management Approach, provides your firm’s “best offer” for the Technical/Management Approach.

RFI #2 – Please provide contact information for Steris rep so bidders who move to Phase 2 can acquire pricing for booms and integrated ceiling system that are to be provided by Contractor.

VA Response:

The local Steris Contact would be:

Lee Jacobsen, Senior Account Manager

(916) 730-2029 Lee_Jacobsen@steris.com

The Local Area Engineer for Steris:

Jay Chernault PDM Southwest Region 714-743-0837 Jay_Chernault@steris.com

RFI #3 - Appendix A specifies new walls shall be a prefab system comprised of lead shielded powder coated stainless steel panels. Please confirm this intent / if confirmed if these walls are part of a Steris provided product, or if VA has any other preferred wall panels vendor.

VA Response: Prefabricated walls shall be MEDGlas Wall EGS0H (MG-010) by MEDIK/Steris.

Make provisions in pre-fab wall to accommodate current monitor(s).

Colors to be picked by the VA.MedGlas specifications included in RFI #3 Response.

mailto:Lee_Jacobsen@steris.com mailto:Jay_Chernault@steris.com

RFI#4 – Appendix A specifies mural on West wall panels. Please confirm this mural will be non-illuminated and direct applied to the wall panels, and please specify approximate mural dimensions.

VA Response: The mural shall be a backlit illuminated integrated MEDglas Wall ESG-H (MGW-

002) by MEDIK/Steris or equal. The image shall be of 300dpi or better. The image shall be selected during the design phase of the project. Approximate mural size shall be 9ft x 15ft. The mural wall cannot be of an open porous finish and shall be cleanable using current detergents being used by the VA.

RFI#5 - Appendix A specifies that one new surgical light will have HD camera and microphone to enhance communication between Surgeon and Control personnel. Please clarify how these signals are desired to be used towards this end i.e., any video integration system as part of, or in addition to any provided by GE, and if in addition to, please identify any VA preferred vendor or required functionality of any new video integration system.

VA Response: The GE supplied video integration system allows third party cameras and other equipment to connect and display on the GE supplied Monitors. The Surgical light boom with require the necessary cabling to connect the camera with the GE Video Equipment. The Camera and microphone shall be by Steris or equal. Equipment will be used by the Surgical personnel in the Operating Suite of OR4.

RFI#6 - Per site walk that occurred 12-16-2021, it was determined that appropriate 480V power source for new GE equipment is not currently identified. Can VA please identify source where power can expect to be derived as location of this power will have a relevant impact on electrical pricing and may require other scopes to support a new power run.

VA Response: See attachment FR-01-03PW_480v-Third Floor. Power source will be from the main electrical room in the basement of Building 1 or from panel ''Z'' on the second-floor room 2A07A, Building1. The contractor will be required to use a qualified Electrical Engineer to determine which power source has capacity to meet the requirements of the GE system and to upgrade the source if required to meet the project demands.

RFI #7 – An RFI form was not found in the attachments.

VA Response: See attached RFI form.

RFI #8 – Reference SOW page 5B; what is the list of VA supplied equipment?

VA Response: The VA has purchased the GE ALIA 740 Interventional Unit that comes with the required support equipment to operate the Unit. Also included will be one Large Display Monitor, 2-19” monitors, Seismic Anchorage System, and the Magnus Maquet Operating Table. These items to be installed by GE and Magnus.

RFI #9 – Where is the electrical panel for a source of 480v power?

VA Response: See RFI response #6.

RFI #10 – Reference SOW page 7I; evaluate current electrical supply to operating rooms. Is this a requirement for all operating rooms or just OR #4 being modified?

VA Response: The current electrical supply in OR #4 is 208v and will require an upgrade to 480v along with adding 480v to the equipment room. The requirement is for OR#4, the new control room, and the new equipment room. This will need to be evaluated by a qualified Electrical Engineer and meet NFPA and VHA Design Standards for electrical as well as the GE requirements for the ALIA 740.

RFI #11 - Reference SOW page 4E; all As Built Auto CAD facility drawings will be made available to the awarded contractor. Can those documents be made available for bidding purposes? If not, can a list of plans and documents be provided that will be made available?
