Question 2: Is there an inspection report template the VA would like to be used?

Answer 2: Pages 5-7 of the attached Blank Inspection Checklist Template is a form your company can use. Also, the attached excel document is an example format for documenting discrepancies.

Question 3: In regards to the past performance survey, will offerors be excluded if our current/past clients do not wish to participate?

Answer 3: In accordance with FAR 52.212-2 EVALUATION—COMMERCIAL ITEMS, starting on page 39 of the solicitation, Offers will be evaluated by performing a comparative evaluation in accordance with FAR 13.106-2(b)(3) to determine which response represents the “best” as a whole. The purpose of the past performance evaluation assesses the level of expectation the Government has in an offeror’s ability to meet the requirements of the solicitation, based on a demonstrated record of performance. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown past performance.
