a) The purpose of this project is to replace the existing landscape plantings, finishes and irrigation systems in selected areas of the campus of the VA Sierra Nevada Health Care System (VASNHCS) Medical Center located at 975 Kirman Avenue, Reno, Nevada 89502. The existing landscaping affected by this work is situated between the sidewalk and Emergency Department building at mid-block on Locust Street, consisting of new planting/finish area which will require new irrigation systems, and at the corner of E. Taylor Street and Kirman Avenue, consisting of new and existing planting/finish areas which will require modification of existing irrigation systems. The existing landscaping areas affected by this work currently serve as transition spaces for patients to move from their vehicles to the campus sidewalks, as well as providing physical security force protection in selected locations, and to create a unified and consistent appearance throughout the facility campus.
b) Estimated Construction Contract Price (design-to limitation): $25,000.00
c) All existing buildings will be occupied during all construction activities. Appropriate Interim Life Safety Measures (exits, etc.) and Infection Control Risk Assessments must be considered and a Safety Management Plan shall be implemented by the contractor.
d) The contractor shall provide all personnel, materials, and equipment necessary to complete the work in accordance with this Statement of Work, the drawings, and VA design criteria as listed in VA design guides and manuals, including the VA Physical Security Design Manuals, which can be found on the internet at the VA Office of Construction & Facilities Management Technical Information Library (TIL) at the following links:

2. SCOPE
a) The work requires the removal of existing landscaping at the sloped area adjacent to the Emergency Department building on Locust Street, extending westward to the sidewalk, northward to the ambulance driveway, and southward to the patient parking exit driveway, consisting of approximately 3200 SF of landscaped area, and removal of selected road surface and landscaping at the ambulance driveway and near the Locust Street bus stop. This shall include but is not limited to the demolition of existing decomposed granite (DG), asphalt concrete (AC) paving, turf, and excavation of trenches to carry irrigation plumbing, and the salvage, relocation, and temporary on-site storage of river rock. The contractor shall provide all work to research and secure the effected systems in the facility in such a manner that limits the work to the landscaping areas and related equipment identified in this scope. The contractor shall control all demolition debris, scrap, waste and other materials generated from the demolition process in a manner consistent with all local, state and federal requirements. All material generated as part of the demolition process shall be removed from the facility to a dumpster/container in a manner consistent with the existing medical center infection control policies.
b) The work will require the removal of existing landscaping at the area near the intersection of E. Taylor Street and Kirman Avenue, between the sidewalks and the curb/on-street parking lanes, extending westward on E. Taylor Street to the walkway between the Specialty Clinic and Community Living Center (CLC), and extending southward on Kirman Avenue to the driveway between the Specialty Clinic and the VASNHCS facility warehouse, consisting of approximately 1600 SF of landscaped area. This shall include but is not limited to demolition of turf, sprinkler heads, and partial demolition of irrigation piping. All material generated as part of the demolition process shall be removed from the facility to a dumpster/container in a manner consistent with the existing medical center infection control policies.
c) The work will require the installation of new plants and five (5) trees, irrigation system, DG, and twelve (12) landscaping boulders measuring at least 36” L x 36” W x 36” H, and reinstallation of salvaged river rock at the sloped area adjacent to the Emergency Department building on Locust Street as described in section 2.a. above. This work shall include, but is not limited to modification of existing irrigation plumbing and control systems, installation of a precast concrete traffic-rated utility trench, irrigation plumbing piping, trenching backfill, irrigation emitters, DG at new planting locations; the contractor shall modify the existing irrigation water supply and controls systems to provide coverage for new planting areas along Locust Street. The contractor shall provide all work to select and install plants and trees that are drought resistant and well-suited to a high desert climate, and which will create a unified and consistent appearance with the other landscaped areas on the facility campus; to this end, contractor shall coordinate plant selections for this project with those listed in the current documentation on file for Office of Construction and Facilities Management Project Number VA-101-11-RP-0103 VA Sierra Nevada Health Care System – Building 1 Seismic Upgrade & Clinical Expansion. This location includes approximately thirty-eight (38) existing boulders and nine (9) trees, as well as an existing sprinkler system. Existing boulders serve as physical security force prevention barriers and shall remain in place; existing trees shall likewise remain in place. The existing sprinkler system shall be modified to remove sprinkler heads, and provide local irrigation emitters at new planting locations. Planting locations will be established around existing trees, and those areas not designated as planting areas shall receive walkway pavers to create a consistent and stable walkway surface between the existing curb/parking lane and the existing sidewalk, and shall be selected to integrate with existing finishes and materials on the VASNHCS campus to provide a unified and consistent appearance. The contractor shall provide all work to select and install plants that are drought resistant and well-suited to a high desert climate, and which will create a unified and consistent appearance with the other landscaped areas on the facility campus; to this end, contractor shall coordinate plant selections for this project with those listed in the current documentation on file for Office of Construction and Facilities Management Project Number VA-101-11-RP-0103 VA Sierra Nevada Health Care System – Building 1 Seismic Upgrade & Clinical Expansion. The contractor shall provide a minimum of three (3) species of plants. Where pavers are to be installed, the contractor shall provide a prepared substrate as described in section 2.e. below. The contractor shall control all construction debris, scrap, waste and other materials generated from the construction process in a manner consistent with all local, state and federal requirements. E.2 52.209-5 CERTIFICATION REGARDING RESPONSIBILITY MATTERS (OCT 2015)
(a)(1) The Offeror certifies, to the best of its knowledge and belief, that—
(i) The Offeror and/or any of its Principals—
(A) Are [ ] are not [ ] presently debarred, suspended, proposed for debarment, or declared ineligible for the award of contracts by any Federal agency;
(B) Have [ ] have not [ ], within a three-year period preceding this offer, been convicted of or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public (Federal, State, or local) contract or subcontract; violation of Federal or State antitrust statutes relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, violating Federal criminal tax laws, or receiving stolen property (if offeror checks "have," the offeror shall also see 52.209-7, if included in this solicitation);
(C) Are [ ] are not [ ] presently indicted for, or otherwise criminally or civilly charged by a governmental entity with, commission of any of the offenses enumerated in subdivision (a)(1)(i)(B) of this provision; and
(D) Have [ ], have not [ ], within a three-year period preceding this offer, been notified of any delinquent Federal taxes in an amount that exceeds $3,500 for which the liability remains unsatisfied.
(1) Federal taxes are considered delinquent if both of the following criteria apply:
(i) The tax liability is finally determined. The liability is finally determined if it has been assessed. A liability is not finally determined if there is a pending administrative or judicial challenge. In the case of a judicial challenge to the liability, the liability is not finally determined until all judicial appeal rights have been exhausted.
(ii) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.
