DRAFT STATEMENT OF WORK (SOW):

DRAFT STATEMENT OF WORK (SOW)

1. Contract Title. American Lake B200 Roof Drainage Repair

2. Background.

Building 200 at American Lake is quite new but was designed poorly. The gutter system directs the water towards the building and the gutter itself is against the side of the building. This had the effect of voiding the 10 year materials warrantee and further caused the majority of the gutters to prematurely rust through. At this location, this has caused a leak into the B-pod hallway as well as into B116.

3. Scope.

This work is restricted to 2 sections of roof above both the hallway in C-pod as well as room B116. It will also address a design flaw in the gutter system but again will be restricted to a max of roughly 100 feet of gutter.

4. Specific Tasks.

a) Remove all existing roofing material and insulation down to the cement deck. Dispose of the material properly.

b) Install new insulation with coverboard and ensure proper slope/taper for drainage.

c) Install new torch-down roofing according to manufacturer's instructions – ensuring to include all materials, flashings, sealants, fasteners, and accessories needed to meet installation requirements and ensure proper watertightness.

d) Manufacturer representative to inspect and issue report verifying system installed properly and maintains warrantee.

e) Remove limited portion of damaged gutter system.

f) Install small section of improved gutter system

g) Perform 1 hour water test

5. Task 1 - Enterprise Management Controls.

Deliverables: Contract Management Plan

Deliverables: Pictures of each stage with multiple views and of trouble/issues discovered during demo of old roofing.

6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

No government equipment will be furnished. The contractor shall provide the equipment needed both to access and repair these roofs. This equipment will be utilized with harnesses and fall protection. It shall be stored at the end of each day or at each break in such a way that patients have no ability to utilize it. Furthermore it shall be stored at each day’s end on the far side of the building so that it is not visible from the entry road to the building.

7. Inspection and Acceptance Criteria.

Observance of methods used, materials purchased, and pictures of each step of the job. Final inspection will use a water test with inspection of interior 24 hours following the test.

8. Risk Control Safety harnesses will be worn by all on the roof and equipment used to get to the roof. The equipment will be used well away from patient areas and stored in such a manner that patients and staff cannot access or use it. Work required to be done above the front entrance will be done both from the side and during hours of little to no traffic.

9. Place of Performance.

All work will be performed on the exterior of Building 200

10. Period of Performance.

20 day following the award of the contract.
