Field Service Report
Summary
Purpose: The Department of Veterans engaged the services of Eaton Electrical Engineering Services and Systems to provide maintenance testing at the VA Medical Center in Boise, ID.
Testing was performed by Greg Gould and Larry Neddo with EESS. All equipment has been inspected and tested to ensure they meet manufacturer’s specifications and industry standards for safe and proper operation.
COMMENTS PERTAINING TO EQUIPMENT TESTED:
Building 67, 67-GDG1 and 67-NDG1. Main bonding jumper was mounted on the wrong side of the neutral sensor. Corrected by EESS.
WORK PERFORMED:
Maintenance testing on low and medium voltage electrical equipment.
REPORT PREPARED BY:
Greg Gould
Test Procedures:
MEDIUM VOLTAGE VACUUM BREAKER
Visual and Mechanical Inspection Inspected breaker for physical damage and installation.
Performed mechanical operation, checked contact wipe and erosion indicators in accordance with manufacturer’s instructions.
Checked tightness of accessible electrical connections.
Inspected breaker to cell alignment and proper grounding.
Checked racking mechanism Cleaned breaker frame, operating mechanism and insulating surfaces.
Electrical Tests Verified proper electrical closing/tripping operation by activation of auxiliary devices.
Performed insulation resistance test phase-to-ground, phase-to-phase and across open contacts.
Performed vacuum bottle integrity test on each pole in accordance with manufacturer’s instructions.
Measured contact resistance.
Performed over-potential test on each phase.
Field Service Report
GROUND FAULT PROTECTION SYSTEM
Visual and Mechanical Inspection Inspected for physical damage.
Inspected neutral main bonding connection to assure:
Proper size.
Zero sequence and residual system were grounded upstream of sensor.
Ground connection was made ahead of neutral disconnect link.
Verified ground electrode conductor for proper size and termination.
Inspected sensors for proper installation.
Verified proper ground fault relay circuit identification.
Electrical Tests Measured pickup current by primary injection at sensor.
Measured time delay above pickup current level by injecting current into the sensor.
Measured system neutral insulation resistance.
PROBLEM SUMMARY:
Building 27, NMDP1 Main. There is a neutral sensor wiring issue when it is connected to the other main and tie. Since the neutral does not have any loads, this issue is low priority.
Transformers T3, T4 and T19 have significant corrosion and should be replaced.
Transformer T2 DGA shows that carbon monoxide and carbon dioxide are high. This indicates significant overheated cellulose insulation. The total dissolved combustible gas levels exceed normal concentrations. We recommend quarterly oil samples and monitoring the transformer for overloading.
Transformer T10N DGA shows that carbon monoxide and carbon dioxide are high. This indicates significant overheated cellulose insulation. The total dissolved combustible gas levels exceed normal concentrations. We recommend quarterly oil samples and monitoring the transformer for overloading. The IR section of the report also shows that the H1 and H3 connections are hot and should be investigated during the next available opportunity.
Transformer T16N DGA shows that hydrogen, methane and ethane levels are high. This indicates that there is possible partial discharge activity and severe overheating. Also carbon monoxide and carbon dioxide are high. This indicates significant overheated cellulose insulation. The total dissolved combustible gas levels exceed normal concentrations. We recommend quarterly oil samples and monitoring the transformer for overloading.
Pad Mounted Switch S15N Load Break Elbow T4C is slightly hotter when compared to similar connections indicating a possible dirty or poor connection. Investigate connection during next available opportunity.
Transformer T26E X3 bushing more than 10 deg. hotter than other two bushings (X1 & X2).
Scan seems to indicate an issue with the internal connection of the bushing. We recommend shutting down the transformer draining the oil and checking for loose connections inside the transformer as soon as possible.
