About this file
This document is a Lead-Based Paint (LBP) Inspection Report for Building 16 at the VA Puget Sound Healthcare System's American Lake Division, conducted on May 4, 2009, by Cole & Associates Training & Consulting, Inc. The comprehensive inspection utilized an X-Ray Fluorescence (XRF) device to test various interior and exterior surfaces for lead-based paint, covering 29 rooms across two floors of an administrative building constructed in 1932.
The inspection results revealed lead-based paint present on multiple components across different rooms, including concrete walls, metal door casings and jambs, wood window frames, window casings, and window stools in various colors such as beige, blue, white, and yellow. The testing followed HUD guidelines and federal regulations (40 CFR 745), with lead concentrations measured in milligrams per square centimeter. While lead-based paint was detected in numerous locations, the report specifically noted that no lead-based paint hazards were identified in the building, and all testing was conducted with proper calibration and quality control procedures.

EXECUTIVE SUMMARY
Cole & Associates Training & Consulting, Inc. was retained by The Department of Veterans Affairs to conduct Lead-Based Paint Inspections of select buildings located at the American Lake Division of the VA Puget Sound Health Care System, 9660 Veterans Drive SW, Tacoma, WA, 98493.
This Lead-based Paint Inspection was conducted in Building 16 on May 4th, 2009 and included XRF testing of each component with a distinct painting history for the presence of lead-based paint.
Robyn Bates, a Certified Lead Inspector arrived on site at approximately 9:00 am on May 4th, 2009, and conducted a preliminary visual survey of the property noting the layout of the structure and room locations.
The building was built in 1932 and is a two-story building used for administrative offices.
The building has twenty-nine rooms.
The Lead Paint testing was conducted utilizing an X-Ray Fluorescence (XRF) device in accordance with the documented methodologies and quality control procedures outlined in Chapter 7 of the U.S. Department of Housing and Urban Development (HUD) Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing, 1997 revision, and the September 24, 2004, Edition No. 1 NITON Performance Characteristic Sheet (PCS).
40 CFR 745 and WAC 365-230-210 defines “Lead Based Paint (LBP)” as any paint or other surface coating (i.e. paint, varnish, shellac, etc.) that contains lead equal to or in excess of 1.0 milligrams per square centimeter (1.0 mg/cm2), or more than 0.5 percent by weight.
All testing was conducted using documented methodologies that incorporated adequate quality control procedures.
As a result of the XRF testing, it is hereby stated that one or more of the components in Building 16 tested POSITIVE for lead-based paint.
SCOPE OF SERVICES
The purpose of this Lead-Based Paint Inspection was to determine the presence of lead-based paint in the structure.
Utilizing an X-Ray Fluorescence (XRF) device, the assessor tested each component with a distinct painting history for the presence of lead based paint. SURFACE BY SURFACE LEAD INSPECTION
Painted surfaces were assessed to identify any friction, impact, and/or visibly deteriorated surfaces.
Before testing, the XRF was calibrated following the XRF Performance Characteristic Sheet (PCS) and the manufacturers’ instructions. The results were recorded on Form
7.2 (Calibration Check Test Results) and were within acceptable tolerances.
Testing was methodically conducted using the XRF clearly notating the location and description of each component tested.
Additional calibration checks were performed at least once every four hours to ensure the XRF stays within acceptable tolerances.
Upon completing the inspection the inspector performed an additional calibration check, and recorded the results on Form 7.2 (Calibration Check Test Results). All calibration results were within acceptable tolerances.
After completing the final calibration tests, the inspector performed a final walk-through making any necessary notes.
Methodology & General References Inspection, sampling, and assessment procedures were performed in general accordance with the guidelines published by the EPA in 40 CFR Part 745, Chapter 7 of the U.S. Department of Housing and Urban Development (HUD).
The lead inspection consisted of the following major activities: visual inspection and XRF testing of paint.
Rooms or room equivalents (i.e. hallways and exterior areas) were numbered.
Walls in those rooms are identified as A, B, C, and D. The “A” wall in each room is the wall in that room which is closest to the street. The remaining walls, “B”, “C”, and “D” are located in ascending order proceeding clockwise from wall “A”.
The painted surfaces in the rooms are identified as components, which can generally be defined as architectural features of the building. Components consist of walls, ceilings, floors, doors, doorjambs, window sashes, windowsills etc. Ms. Bates tested each visible interior and exterior component with a distinct paint history. The component location, test location, and test results, expressed in terms of mg/cm2, for each component tested are recorded on the attached Lead Based Paint Inspection Testing Data Sheets. Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing, 1997 revision, and the September 24, 2004, Edition No. 1 NITON Performance Characteristic Sheet
(PCS).
Equipment A NITON X-Ray Fluorescence (XRF) device was utilized to perform paint analysis on each of the building components to determine the presence of lead.

LEAD-BASED PAINT INSPECTION SUMMARY
This Lead-Based Paint Inspection was conducted on May 4th, 2009. The inspection included the testing of each component with a distinct painting history for the presence of lead-based paint. All exposed and accessible coated surfaces were tested as requested by the client. Testing was conducted using an X-Ray Fluorescence (XRF) device according to local, state and federal regulations.
This Lead-Based Paint Inspection was conducted in accordance with 40 CFR 745 Lead;
Requirements for Lead Based Paint Activities in Target Housing and Child Occupied Facilities, 40 CFR 745.227(d) Inspection requirements.
The results of this inspection indicate that lead in amounts greater than or equal to 1.0 mg/cm² in paint was found on one or more of the building components, using the inspection protocol in Chapter 7 of the HUD Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing (1997). However, some painted surfaces may contain levels of lead below 1.0 mg/cm², which could create lead dust or lead-contaminated soil hazards if the paint is turned into dust by abrasion, scraping, or sanding. This report should be kept by the inspector and should also be kept by the owner and all future owners for the life of the facility.
VISUAL INSPECTION
A Lead-Based Paint Inspection of the facility was conducted and consisted of:
1. A visual inspection to determine whether the structure is in good condition.
2. Areas that were determined to be in poor condition and areas that the scope of work indicates damage or disturbance to painted surfaces were tested further.
All testing was conducted using documented methodologies that incorporated adequate quality control procedures.
LEAD BASED PAINT HAZARD ASSESSMENT
40 CFR 745 defines a “Lead Based Paint (LBP) Hazard” as any condition that causes exposure to lead from lead contaminated dust, lead contaminated soil, or lead contaminated paint that is deteriorated or present in accessible surfaces, friction surfaces or impact surfaces that would result in adverse human health effects pursuant to the Toxic Substances Control Act, Section 403.
As defined by 40 CFR 745 a “Lead Paint Hazard” is any of the following:
A. Any lead based paint on a friction surface that is subject to abrasion;
B. Any damaged or otherwise deteriorated lead based paint on an impact surface that is caused by impact from a related building component;
C. Any chewable lead based painted surface on which there is evidence of teeth marks and;
D. Any other deteriorated lead based paint in any residential building or child occupied facility or on the exterior of any residential building or child occupied facility. Lead paint hazards were identified at this facility. (40 CFR 745.65(a)).
No Lead-based Paint Hazards were identified in this building.
