B.4 STATEMENT OF WORK

STATEMENT OF WORK

A. GENERAL

A.1 GOAL:

The objective of this contract is to provide set up, assembly and installation audio video conferencing system procured from the vendor. The vendor will dispose of all packing material and debris that stem from the delivery and installation per each delivery made. The vendor shall provide shipping and the equipment necessary to offload the items from the delivery trucks. The delivery truck is required to have a lift gate since there may or may not be access to a loading dock at the installation site. See B.4 for specific requirements.

A.2 BACKGROUND:

The VA SORCC projected dates for activating 207A Residential Meeting room building on the dates to be decided.

B. TECHNICAL APPROACH/TASK

B.1 HOURS OF OPERATION:

Contractor to work at VA SORCC between the hours of 8am and 4:30pm, Monday through Friday, excluding federal holidays.

B.3 METHODS TO BE USED:

The vendor shall use standard methods and procedures for the items required to be delivered and installed to the VA SORCC. All work shall be in conformance with State, local and/or Federal laws.

B.4 SPECIFIC REQUIREMENTS:

Installation dates shall be determined after the contract has been awarded. These dates are subject to change depending on clinic and staff availability. Vendor shall have the ability to store all items related to this procurement for a 90-day period from the initial installation date.

Contracting Officer will notify vendor of the contact information of VA SORCC upon award of the contract. The selected vendor shall deal directly with the shipping and installation contractors for this procurement. One POC shall be required from the selected vendor for all contact with VA SORCC personnel.

System Overview and Function

All rooms will be built on a centralized AV distribution system. The system is considered an advanced room control and distribution, that allows users to connect to and operate a variety of technology, controlled by a user interface in each space. Each space will be able to be operated independently or combined. This means that each system can share content via video and audio to each of the other spaces. Audio and video in the divisible spaces will be combinable with microphones as well. The user will have the ability to operate the systems for presentation of content locally, audio conferencing, and web based video conferencing. Users will be able to connect to present in multiple ways.

Multipurpose Room (105) - (2) Way divisible) Seating for 500

Overview and Function:

This room will be considered an advanced room that allows users to connect to and operate a variety of technology, controlled by a user interface. The user will have the ability to operate the system for presentation, audio conferencing, and web-based video conferencing. Users will be able to connect to present in multiple ways. The room is divided in half and a screen will be visible on either side. While the room is in combined mode, the projectors will show the same content. In separate mode, they will be able to operate independently. All components of the audio and video system, including video, microphones, speakers and content sources will operate fully together or separate depending on the mode of the room.

Contractor will provide the following Services:

Video:

Provide and install one (1) projector and screen on each half of the room for a total of two (2) projectors systems

Provide and install one (1) HDMI port for a hardwired connection on the wall at either side of the room for a total of two (2) HDMI connections.

Provide and install one (1) wireless content sharing device for each of the rooms for a total of two (2) devices.

Integrate with customer furnished CFE cable TV Tuner, audio, and microphones.

Provide and install speakers in the ceiling. Speakers will be utilized within conferences and for general voice lift.

Provide and install eight (8) microphones for the space. These will be a mix of handheld and LAV wireless microphones. Vendor will provide and install four (4) handheld microphones. Vendor will install four (4) wireless LAV microphones.

Vendor will provide and install microphone antenna and audio DSP upgrade to the system. This DSP will replace existing system.

Conferencing:

Provide and install one (1) PTZ camera on either side of room for a total of two (2) cameras. These will be tied back to the local PCS for web-based conferencing.

Stage/Classroom 3 (106)

This room will be considered an advanced room that allows users to connect to and operate a variety of technology, controlled by a user interface. The user will have the ability to operate the system for presentation, audio conferencing, and web-based video conferencing. Users will be able to connect to present in multiple ways. This room will be an extension of the multi-purpose space, effectively being a third divisible space based on the controls of the touch panel. While the area is in combined mode, the projectors/display will show the same content.

The classroom’s display will function as a confidence monitor for the multi-purpose space. In separate mode, the room will be able to operate independently. All components of the audio and video system, including video, microphones, speakers and content sources will operate fully together or separate depending on the mode of the room.

Contractor will provide the following Services:

Provide and Install one (1) 98” Display with Wall Mount

Provide and install one (1) HDMI port for a hardwired connection to the wall.

Provide and install one (1) wireless content sharing device

Integrate with customer furnished CFE Cable TV tuner, audio and microphones.

Audio and Microphones:

Provide and install speakers in the ceiling. Speakers will be utilized within the conference and for general voice lift.

Provide and install two (2) microphones for the space. These will be a mix of handheld and LAV wireless microphones.

Provide and install microphone antenna and audio DSP upgrade to the system. This DSP will replace existing system.

Provide and install one (1) PTZ camera at the front of the room. This will be tied back to the local PCS for web-based conferencing.

Class Room 2 (108)

This room will be considered an advanced room that allows users to connect to and operate a variety of technology, controlled by a user interface. The user will have the ability to operate the system for presentation, audio conferencing, and web-based video conferencing. Users will be able to connect to present in multiple ways. In admin mode, users will be able to receive video and audio content from all rooms.

Contractor will provide the following service:

In admin mode, users will be able to receive video and audio content from all rooms.

Contractor will provide the following service:

Provide a one (1) year system warranty on workmanship

Provide user manuals in PDF format and printed with graphics and call outs for common use of system.

Provide training for local IT staff on basic system use.

B.6 ID BADGE:

Contractor shall wear an identification badge at all times while on station. Badge will be obtained in building 229 when signing in and shall be returned upon completion of the services when signing out.

B.7 PERSONNEL:

The contract manager shall be responsible for management and coordination of the contract and shall be the point of contact with all government representatives. The contractor shall notify the contracting officer in writing prior to contract start date the names of the Contract Manager and an alternate contract manager.

The Contract Manager shall be available for the Government to reach. During normal business hours, the contract manager shall respond within one business day from the time of notification.

During non-business hours, the Contract Manager shall respond within one business day.

B. 8 SERVICES DELIVERY SUMMARY:

The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

C. SPECIAL CONTRACT REQUIREMENTS

C.1 QUALITY CONTROL:

The contractor is responsible for developing and maintaining a quality program to ensure that the services are performed in accordance with commonly accepted commercial practices. The contractor is responsible for developing and implementing procedures to identify, prevent, and ensure non-recurrence of defective services. At a minimum, the contractor is responsible for developing quality control procedures addressing each specific bid item of the contract to include a service delivery summary. The contractor is responsible for submitting the quality control program to the Contracting Officer with their proposal.

C.2 QUALITY ASSURANCE:

1. The government will periodically evaluate the contractor’s performance by way of

Environment of Care Specialist to monitor the performance to ensure services are received.

2. Contractor will be here on the appointed times and days, unless necessary to come more often as requested by the VA SORCC.

3. The completed services shall be performed in an efficient and reliable manner. The VA

SORCC has the right to reject work if the work is not performed in an efficient and reliable manner with the timelines and frequencies outlined above.

C.3 GOVERNMENT REMEDIES:

The Contracting Officer is responsible for the Contract Terms and Conditions.

C.4 RECOGNIZED HOLIDAYS:

The contractor is not responsible to provide services on the following days but must provide frequent enough service prior to and after these holidays:

New Years Day, Martin Luther Kings Day, President’s Day, Memorial Day, Juneteenth Fourth of July, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas.

C.5 PERSONNEL IDENTIFICATION:

C.6 PERSONNEL POLICY:

The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel:

1. Worker’s compensation, Professional Liability Insurance, Income tax withholding, and

Social Security payments.

2. The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor.

C.7 LAWS & ORDINANCES:

The Contractor shall comply with all applicable laws, ordinances and regulations (Federal, state, city or otherwise) covering his operations under the contract.

C.8 TWO WAY RADIOS:

The use of two-way radios is prohibited in all VA SORCC buildings. The equipment shall be turned off upon entering either of those buildings.

C.9 INVOICES:

All payments shall be made in arrears upon receipt of a proper invoice. Invoices will provide the following minimum information:

C.3 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING—

MONITORING AND COMPLIANCE (JUL 2018)

(a) This solicitation includes VAAR 852.219-10 VA notice Of Total Service -disabled Veteran Owned Small Business Seat-Aside (Jul 2019) (Deviation)

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

(c) All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.

(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

C.4 VAAR 852.247-73 PACKING FOR DOMESTIC SHIPMENT (OCT 2018)

Material shall be packed for shipment in such a manner that will insure acceptance by common carriers and safe delivery at destination. Containers and closures shall comply with regulations of carriers as applicable to the mode of transportation.

C.5 52.211-6 BRAND NAME OR EQUAL (AUG 1999)

(a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation.

(b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must—

(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;

(2) Clearly identify the item by—

(i) Brand name, if any; and

(ii) Make or model number;

(3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and

(4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications.

(c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.

(d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
