a. Answers to Vendor Questions.
1. Question:
Can you elaborate on the typical volume, types of casework and frequency typical for the VA Puget Sound Health Care System? (spine vs craniotomies vs vascular vs ENT vs orthopedic surgeries) Answer:
The VA Puget Sound Health Care System performs almost exclusively spine cases.
2. Question:
Some cases are much longer than others and some require variability of intraoperative tests (8 channel EEG, functional cortical mapping, ABR responses, for craniotomies versus EMG, SEP, MEP, 2 Channel EEG for spine, etc.). How should each unit be priced?
Answer:
Pricing shall be priced per as an all-inclusive case in accordance with B.2 Schedule of Services in the RFQ.
3. Question:
Are neuromonitoring supplies stocked at your VA (needle electrodes, corkscrew electrodes, cortical strip electrodes, ball-tip probes, yingling probes, specialty items, etc.) or are these provided by the Contractor?
Answer:
All supplies are provided by the Contractor.
4. Question:
Referring to section 4.4.1.1, it states that the contractor shall be local to the Seattle/Puget Sound area and have more than one backup technician, 3.1.3.2 states that the contractor must be available within 2 hours and E.2.D.1 states to provide a minimum of 5 qualified technologists who reside within 75 miles. Can you please clarify what the preferred requirements are and if there is any flexibility for per diem travel?
Answer:
The Contractor shall have a minimum of 5 technologists who resided within 75 miles of the VA Puget Sound Health Care System, available to provide services. This requirement supports the 2-hour response time to perform emergency IONM cases. Due to historical issues with traveling IONM technologists, there is no flexibility for per diem travel.
b. RFQ Corrections.
1. The first two (2) sentences of PWS Paragraph 4.4.1.1. is updated as follows:
To provide PSHCS on-site complete Intraoperative Neuromonitoring Services to include after-hours coverage in support of PSHCS, 1660 S Columbian Way, Seattle, WA 98108, The Contractor shall have a minimum of five (5) technologists who are local to the Seattle/Puget Sound area, reside within 75 miles of the PSHCS, and are available to provide IONM Services.
2. On Page 5 of Section B.2 Schedule of Services, the minimum and maximum single order amounts are updated as follows:
In accordance with FAR 52.216-19, the minimum single order amount is 5 cases, and the maximum single order amount is 70 cases.
3. Section C.2.2, FAR Clause 52.216-19 Order Limitations is updated as follows:
(a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than 5 cases, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract.
(b) Maximum order. The Contractor is not obligated to honor—
(1) Any order for a single item in excess of 70;
(2) Any order for a combination of items in excess of 70 cases; or
(3) A series of orders from the same ordering office within 10 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section.
(c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section.
(d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 3 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source.
